Antibiotic prophylaxis for the prevention of staphylococcus epidermidis infections in orthopedic surgery: are first or second generation cephalosporins adequate?

Abstract

An animal model using joint replacement materials was used to compare the efficacy of cefazolin to that of vancomycin/ceftazidime combination in preventing infection from Staphylococcus epidermidis. Forty four New Zealand rabbits had a cobalt-chrome with methacrylate bone cement in the distal femoral metaphysis. Deliberate contamination of the wound with one or two strains of Staphylococcus epidermidis was performed with or without the administration of prophylactic antibiotics and the infection rates compared. Cefazolin prevented infection in only 14% of the animal compared to 86% protected by the combination of vancomycin and ceftazidime. The results were compared with both methicillin sensitive and resistant strains. Epidemiological data and the results of our experiments suggest that the combination of vancomycin and ceftazidime should be considered for contemporary prophylaxis of infection in total joint replacement.
