Sodium nitroprusside infusion for controlled hypotension during anesthesia for spinal surgery

Öz

In this study we investigated, the effects of sodium nitroprusside (SNP) infusion which Was given contralIed hypotension (CH) during anesthesia for spinal surgery, on hemodynamics and the amount of blood 1055. 33 female and 17 male patients (age range 14-25 years) who were in 11. ASA classification were studied. SNP infusion (1-6 ,µg/kg/min) Was starled 10 minutes before the procedure that needed CH. During SNP infusion mean arterial pressures (MAP) were decreased to two thirds of control vaIues. MAP's were not allowed to drop below 60 mmHg. After SNP infusion had been stopped, MAP s rised to 88% of their control vaIues in 4.2±2 minutes. During CH with SNP, the amount of blood loss Was 35 % Iess than the spinal operations done without CH. It is concluded that because its hemodynamic effects are controlled easily and reversed to control values in a few minutes after its infusion is stopped SNP can be used for CH during anesthesia for spinal surgery.

Öz

Çalışma spinal cerrahi anestezisinde kontrollu hipotansiyon (KH) yaratmak amacı ile kullanılan ajanlardan biri olan sodyum nitroprussid'in (SNP) hemodinomi ve kanama miktarına etkilerini araştırmayı amaçlamıştır. Çalışmamız ASA risk sınıflaması II. gruptan yaş aralığı 14-25 olan 33 kadın, 17 erkek toplam 50 olgu üzerinde yapılmıştır. KH gerektiren uygulama başlamadan yaklaşık 10 dakika önce SNP infüzyonuna 1-6 ,µg/kg/dk doz aralığı ile başlanmıştır. SNP uygulaması ile ortalama arter basıncı (OAB) kontrol ölçümlerinin 2/3'üne kadar düşürülmüştür. Kontrolde esas alınan OAB'nin 60 mmHg altına düşmemesi sağlanmıştır. SNP infüzyonu kesildikten 4.2±2 dakika sonra OAB kontrol değerlerinin %88'ine yükselmiştir. Çalışmamızda kanama miktarı KH uygulanmayan vakalara göre %35 oranında az bulunmuştur. SNP infüzyonu kesildikten kısa bir süre sonra hemodinamik değerlerin kontrol değerlerine yükselmesi ve etkisinin kolay kontrol edilmesi nedeniyle SNP'nin spinal cerrahide KH uygulamasında kullanılabilecek bir ajan olduğu kanısına varılmıştır.
