Günübirlik Cerrahi Hastalarının Ameliyat Öncesi Dönem Kaygı Düzeylerinin Ameliyat Sonrası Ağrı ve Derlenme Kalitesi ile İlişkisi: Tanımlayıcı Çalışma

Öz

Bu araştırma, günübirlik cerrahi hastalarının ameliyat öncesi kaygı düzeylerinin postoperatif ağrı ve derlenme kalitesi üzerine etkisini belirlemek amacıyla yapılmıştır. Tanımlayıcı türdeki çalışma, bir üniversite hastanesinde günübirlik cerrahi geçiren 105 hasta ile gerçekleştirildi. Veriler, Aralık 2018- Haziran 2019 tarihleri arasında sosyodemografik ve klinik özellikleri içeren “Kişisel Bilgi Formu”, “Ameliyata Özgü Kaygı Ölçeği”, “Sayısal Ağrı Ölçeği”, “Derlenme Kalitesi-40 Ölçeği” kullanılarak elde edildi. Hastaların ameliyata özgü kaygı puanı ortalaması 29,00 ± 8,32 olarak bulundu; altıncı saatteki ağrı puanı 6,00 ± 1,99, onuncu saatteki ise 3,00 ± 1,81 olarak saptandı. Derlenme Kalitesi-40 toplam puanı 158,00 ± 18,91 ile yüksek düzeyde bulundu. Bilgilendirme yapılan hastaların kaygı düzeylerinin anlamlı biçimde daha düşük olduğu (p=0,008<0.05), değişen uyku düzeninin ve kaygının derlenme kalitesini azalttığı görüldü (p=0,001<0,05). Kaygı ile ağrı arasında pozitif yönde (r=0,342, p<0,01), kaygı ile derlenme kalitesi arasında ise negatif yönde ilişki saptandı (r=–0,409, p<0,001). Bu araştırmada preoperatif kaygı düzeyi orta düzeyin üzerinde, postoperatif ağrı düzeyi orta ve derlenme kalitesi yüksek bulundu. Preoperatif kaygının arttıkça ağrının arttığı ve derlenme kalitesinin azaldığı saptandı.

Araştırma için Düzce Üniversitesinin Girişimsel Olmayan Klinik Çalışmalar Etik Kurulundan (Tarih: 13.02.2019-Karar no:35) ve araştırmanın verilerinin toplanacağı kurumdan (30.05.2019 tarihli, 13505 sayılı) izin alındı. Araştırmaya katılmayı kabul eden hastalardan, Helsinki Bildirgesine uygun olarak araştırmanın amacı, katılımın gönüllü olduğu, araştırmaya katılmaları halinde kişisel bilgi ve gizliliğin korunacağı gibi konular kendilerine açıklandıktan sonra yazılı ve sözlü onam alındı.

The Effect of Preoperative Anxiety Levels of Day Surgery Patients on Postoperative Pain and Recovery Quality : Descriptive Study

Öz

This study was conducted to determine the effect of preoperative anxiety levels on postoperative pain and quality of recovery in ambulatory surgery patients. This descriptive study was conducted with 105 patients undergoing ambulatory surgery at a university hospital. Data were collected between December 2018 and June 2019 using a Personal Information Form covering sociodemographic and clinical characteristics, the Surgery-Specific Anxiety Scale, the Numerical Pain Scale, and the Quality of Recovery-40 Scale. The patients' mean surgery-specific anxiety score was 29.00±8.32; the pain score at the sixth hour was 6.00±1.99 and at the tenth hour was 3.00±1.81. The total Quality of Recovery-40 score was found to be high at 158.00±18.91. Patients who received information had significantly lower anxiety levels (p=0.008<0.05), and altered sleep patterns and anxiety were found to reduce the quality of recovery (p=0.001<0.05). A positive correlation was found between anxiety and pain (r=0.342, p<0.01), and a negative correlation was found between anxiety and quality of recovery (r=-0.409, p<0.001). In this study, preoperative anxiety levels were above moderate, postoperative pain levels were moderate, and recovery quality was high. It was found that as preoperative anxiety increased, pain increased and recovery quality decreased.
