İNFERTİLİTE TEDAVİSİ VE JİNEKOLOJİK KANSER İLİŞKİSİ

Öz

Amaç: İnfertilite oranı gerek Türkiye’de gerekse dünyada gün geçtikçe artmaktadır. Yapılan bu sistematik derleme ile de artan “infertilite tedavisi ile jinekolojik kanserler arasında ilişki” olup olmadığına dair son beş yıl içerisinde yapılmış olan kohort ve vaka-kontrol araştırmalarını incelemek amaçlanmıştır.
Yöntem: Çalışma, 2014-2019 yılları arasında “infertilite tedavilerinin jinekolojik kanserler üzerindeki etkileri ile ilgili yapılmış olan” araştırmaların geriye dönük taranması ile yapılmıştır. Tarama yapılırken Türkçe veri tabanı için “infertilite tedavisi” ve “kanser”, yabancı veri tabanları için “infertility care”, “cancer” anahtar kelimeler kullanılmıştır. Bulunan makalelerin içlerinden jinekolojik olmayan kanserler ile yapılmış çalışmalar, araştırma olmayan derleme çalışmaları ve tez çalışmaları çıkarılmıştır. Araştırmada kohort çalışmalar ile vaka-kontrol çalışmaları alınacak şekilde kısıtlama yapılmıştır. On üç tane kaynak sistematik inceleme için ele alınmıştır. İçlerinden diğer çalışmalarla bütünlük göstermeyen iki çalışma ve tam metni bulunmayan bir çalışma elenmiş, 10 çalışmanın tam metin¬leri kaynak olarak alınmıştır.
Sonuç: İncelenen çalışmalar sonucunda; yardımcı üreme teknikleri (YÜT) tedavileri ile hormona bağımlı jinekolojik kanserlerde (meme, uterus, over kanserleri) hafif artışlar gözlenmiştir bununla beraber bu artışın infertilite durumu ile mi yoksa YÜT tedavileri ile mi ilişkili olduğu tam olarak netlik kazanmamıştır. Bu bağlamda elde edilen bulgular ışığında infertilite tedavisi gören kadınlarda; jinekolojik kanser riski konusunda daha ileri çalışmalar yapılmalı ve YÜT ile tedavi edilenlerin takibinin devam etmesi teşvik edilmelidir.

RELATIONSHIP BETWEEN INFERTILITY TREATMENT AND GYNECOLOGICAL CANCER

Öz

Aim: Infertility rate in both Turkey and the world has increased day by day. With this systematic review, it was aimed to examine the cohort and case-control studies conducted in the last five years regarding whether there is a relationship between infertility treatment and gynecological cancers.
Methods: The study was carried out by retrospective screening of the researches which were done about the effects of infertility treatments on gynecological cancers between 2014-2019. During the screening, the words "infertility treatment" and "cancer" were used for the Turkish database. Keywords “infertility care” and “cancer” were used for foreign databases. Articles made with non-gynecological cancers, reviews and thesis studies were removed. In the study, cohort studies and case-control studies were taken. Thirteen sources were handled for systematic review. Two studies that did not complement with other studies and one study without full text were excluded, and the full texts of ten studies were taken as the source.
Conclusion: The effects of such infertility treatments on gynecological cancers have been investigated in many studies. Although many studies have observed slight increases in hormone-dependent gynecological cancers, it is not clear whether this increase is due to infertility or ART treatments. As a result of the findings, in women receiving infertility treatment; more studies on the risk of gynecological cancer should be done. Follow-up of women treated with ART should be encouraged.
