Obsessional neurosis was first introduced by Freud with some basic components that include inner
thoughts and some compulsive acts. Lacan later used this term as a structure in his subject-formation

approach. According to Lacan, the subject occurs in some important moments which have some different

mechanisms and structures. Obsessional neurosis is one of those structures formed by separation moment.

Indeed, the main feature of that structure is related with the issue of overcoming the anxiety that emerges

from separation through the way of refusing the Other. In the current paper, the obsessional neurosis was

detailed based on Lacanian subject-formation diagnosis approach with some features such as refusing the

Other, desire, guilt, and control. Then, a case was examined within that context by drawing attention to the

relation between his symptoms and previous patterns. At the end, therapy process of the case and Lacanian

therapeutic approach were discussed.
