The Complementary Role Of Dıffusıon-Weıghted Mrı In The Evaluatıon Of Renal Lesions: A Retrospectıve Study

Objective: The aim of this study was to evaluate the diffusion-weighted magnetic resonance imaging (DWI) findings of renal masses and to assess the complementary role and limitations of apparent diffusion coefficient (ADC) measurements in routine clinical practice.
Materials and Methods: This retrospective, single-center study included 50 renal lesions detected on abdominal magnetic resonance imaging examinations performed between 2009 and 2011. Lesions were classified as benign or malignant based on histopathological confirmation or characteristic imaging findings with clinical follow-up. Diffusion-weighted imaging was performed using b-values of 0 and 1000 s/mm², and ADC measurements were obtained by placing regions of interest within the lesions while avoiding necrotic or hemorrhagic areas. ADC values of renal lesions and adjacent normal renal parenchyma were recorded and compared. Interobserver agreement between two radiologists was assessed using the intraclass correlation coefficient (ICC).
Results: Of the 50 renal lesions, 25 were benign and 25 were malignant. Malignant lesions demonstrated significantly lower mean ADC values (1.43 ± 0.73 ×10⁻³ mm²/s) compared with benign lesions (2.76 ± 0.98 ×10⁻³ mm²/s) (p < 0.001). Inflammatory lesions exhibited intermediate ADC values, showing partial overlap between benign and malignant groups. Interobserver agreement for ADC measurements was high.
Conclusion: Diffusion-weighted MRI provides complementary information in the evaluation of renal masses by reflecting tissue diffusion characteristics. However, due to overlapping ADC values among different lesion types, its diagnostic value is limited when used in isolation. ADC measurements should be interpreted together with conventional MRI findings and clinical information in routine practice.
