Retrospective Analysis of Epistaxis Cases: The Impact of Comorbidity and Recurrence on Treatment Approaches

Öz

Objective: The aim was to evaluate the etiology and treatment approach of patients who visited our clinic with nosebleeds, in accordance with the latest literature. Material and Methods: Between January 2018 and March 2023, patients who had been diagnosed with epistaxis and either consulted the outpatient clinic of a tertiary university hospital's ear, nose, and throat department or were referred from the emergency department and had available data through the hospital's information system were retrospectively reviewed. Results: Of the 2169 patients diagnosed with epistaxis, 658 patients aged 0-95 years with no exclusion criteria were included in the study. The average age was 38.4 years, with 54% of the participants being male. The highest number of admissions occurred in February. Recurrent epistaxis was observed in 28.1% of patients, while 8.7% had a single episode, which was more common among women. Most patients presented on an outpatient basis (78.9%) and received only medical treatment (90.4%). A history of surgery was present in 3.3% of cases, mostly following septorhinoplasty. Most 2.6% of the patients who needed hospitalization had other health conditions. The recurrence rates were also higher in patients with comorbid conditions. Conclusion: Epistaxis is a common otorhinolaryngologic emergency that can usually be managed with outpatient medical treatment. However, patients with comorbidities and older age are at a higher risk for recurrence and hospitalization. Careful evaluation of the patient’s medical history and comorbid conditions is essential for optimal management.
