Comment on: “Changes in Serum Nitric Oxide Levels in COVID-19 Patients”

Nitric oxide (NO) plays a central role in endothelial function and penile erection through cyclic guanosine monophosphate–mediated smooth muscle relaxation. Emerging evidence suggests that COVID-19 may adversely affect NO pathways via endothelial dysfunction, potentially contributing to erectile dysfunction (ED). In this letter, we comment on the recently published article “Changes in Serum Nitric Oxide Levels in COVID-19 Patients” and provide clinical perspectives from urological practice. Previous studies have demonstrated reduced endothelial nitric oxide synthase expression and the presence of SARS-CoV-2 within penile vascular endothelial cells in COVID-19–positive patients, supporting a direct vascular impact of the disease. Clinically, impaired sexual function during the COVID-19 pandemic has been frequently reported and appears to be closely associated with both organic endothelial injury and psychological factors such as anxiety and depression. While some data suggest that erectile dysfunction may improve over time following acute infection, the long-term relationship between serum NO levels and erectile function remains unclear. We highlight the importance of further evaluation of erectile function in male patients using validated questionnaires and correlating these findings with previously measured serum NO levels. Such follow-up assessments may provide valuable insights into the vascular and functional consequences of COVID-19 and contribute to a more comprehensive understanding of its urological implications.
