GENEL PEDİATRİ ÜNİTESİNDE AMPİRİK MONO/KOMBİNE ANTİBİYOTİK UYGULAMALARI VE İLİŞKİLİ FAKTÖRLERİN DEĞERLENDİRİLMESİ: KESİTSEL RETROSPEKTİF ÇALIŞMA Evaluation of Empirical Mono / Combined Antibiotherapy Consumption and Related Factors in General Pediatric Ward: A Cross-Sectional Retrospactive Study

Öz

Amaç: Pediyatrik çağda uygunsuz antibiyotik kullanımı 70-80%’lere ulaşabilmektedir. Bu durum ek mali yük ve antimikrobiyal direnç gibi sorunları da beraberinde getirmektedir. Bu çalışmada genel pediyatri ünitesinde ampirik mono/kombine antibiyotik kullanımı ve ilişkili faktörler araştırılmıştır. Gereç ve Yöntem; Çalışmanın kesim noktasından geriye dönük olarak son 8 aylık dönemde toplam yatan hasta sayısına göre ampirik mono/kombine antibiyotik kullanım oranları belirlendi. Yaş, yaş aralıkları (0-<1, 2-<5 ve 6-<18 yaş), ön tanılar, kantitatif C-reaktif protein, beyaz kan hücresi sayısı, eritrosit sedimantasyon hızı ve hastane yatış süresinin mono/kombine antibiyotik tedavi ile ilişkileri incelendi. Bulgular; Antibiyotik kullanımı yatan hastalarda %57,1 (500/875) idi ve bunların %45’ine (394/875) mono, %12,1’ine (106/875) ise kombine antibiyotik tedavi verilmişti. Antibiyotik uygulanan hastaların yaş ortancası 4 yaş (aralık 1-18) ve %21,8’i 0-<1 yaş, %44’ü 2-<5 yaş ve %34,2’si de 6<18 yaş aralığındaydı. Hastaların büyük çoğunluğu solunum yolu (%62), genitoüriner sistem (%17,4) ve gastrointestinal sistem (%8,6) enfeksiyonu tanısı aldı. Eritrosit sedimantasyon hızı (ESH) ortanca 28,5 mm/saat (aralık 2-129), hastane yatış süresi ortanca 4 gün (aralık 1-40) idi. Ampirik mono antibiyotik tedavide en çok penisilinler (%48,4) ve 3.kuşak sefalosporinler (%38,1) tercih edildi. Geniş spektrum tedavi hedeflendiğinde en çok ikili kombinasyonlar (%77,4) kullanıldı. Penisilin+3.kuşak sefalosporin (%28,3) kombinasyonu ise tüm kombinasyonlar içinde en çok tercih edilendi. Regresyon analizinde hastanın küçük yaşta (0-<1 yaş) ve ESH’nın yüksek olmasının kombine antibiyotik riskini yaklaşık 2-3 kat arttırdığı görüldü. Sonuç; Bu çalışmada hasta yaşı (özellikle 0-<1 yaş aralığı) ve yüksek ESH’nın kombine antibiyotik tedavi uygulaması için etkili faktörler olduğu tespit edildi. Genel çocuk servislerinde akılcı antibiyotik stratejileri geliştirmek için tüm süreci kapsayan mevsimsel dönem, hasta ve hastalıkla ile ilgili faktörler gibi birçok değişkeni hesaba katabilen daha gelişmiş yöntemlere ihtiyaç olduğu gözükmektedir. This situation brings problems such as additional financial cost and antimicrobial resistance. In this study, the empirical mono/combined antibiotic consumption and related factors were researched in a general pediatric ward. Material and Methods; This study was carried out retrospectively on the inpatiens from the cut-off point and was determined the rate of empirical mono/combined antibiotic use. The relation of age, age interval (0-<1 year, 2-<5 years and 6-<18 years), preliminary diagnosis, quantitative C-reactive protein, white blood cell count, erythrocyte sedimentation rate and hospitalization duration with mono/combined antibiotherapy were investigated. Results; Of inpatients, 57.1% used antibiotics (500/875) and of these 45% (394/875) used mono and 12.1% (106/875) used combined antibiotherapy. The median age of patients administered antibiotherapy was 4 years (range 1-18) and according to age interval 21.8% were 0-<1 year, 44% were 2-<5 years and 34.2% were 6-<18 years. The majority of patients had respiratory tract (62%), genitourinary system (17.4%) and gastrointestinal system (8.6%) infection diagnosis. Erythrocyte sedimentation rate (ESR) was median 28.5 mm/h (range 2-129) and hospital stay was median 4 days (range 1-40). For empirical mono antibiotherapy mainly penicillin (48.4%) and 3rd generation cephalosporins (38.1%) were chosen. Dual combinations (77.4%) were used at most when broad-spectrum therapy was targeted. Penicillin + 3rd generation cephalosporin (28.3%) combination was the most preferred among all combinations. Regression analysis showed that the risk of combined antibiotic therapy increased by 2-3 times in the younger age (0-<1 year) and high ESR Conclusion; In this study, patient age (especially 0-<1 year range) and high ESR were found to be effective factors for combined antibiotic treatment. It is considered there is a need for more developed methods accounting for many variables like seasonal periods and factors relating to the patient and disease during the whole process to develop rationale antibiotherapy strategies for general pediatric wards.
