Öz

ÖZET Amaç: Erkeklerde üretral darlık en sık bulbar üretrada gözlenmektedir. Bulber üretradaki 2 cm’ den uzun darlıklarda, bukkal mukozal greft yerleştirilen substitüsyon üretroplastide, greftin kullanıldığı yerin cerrahi etkilerini araştırmayı amaçladık. Gereç ve Yöntemler: Kliniğimizde 2 cm’den uzun bulbar üretral darlık nedeniyle dorsal ve ventral greft onlay üretroplasti uygulanan hastaların demografik ve darlık özellikleri retrospektif olarak değerlendirildi. Dorsal ve ventral greft üretroplasti uygulanan gruplar başarı ve komplikasyon oranları açısından karşılaştırıldı. İşeme şikayetlerinin olmaması ve maksimum idrar akış hızın 15 ml/sn’ in üzerinde olması başarı kriteri olarak kabul edildi. Bulgular: Dorsal onlay greft uygulanan 52 (%83,9), ventral onlay greft uygulanan 13 (%81,2) hastada üretroplasti ile başarı sağlandı. Gruplar arasında anlamlı farklılık tespit edilmedi. Demografik ve darlık özelikleri, operatif ve cerrahi sonuçlar ile komplikasyon oranları (%14,5 ve %12,5) açısından iki grup benzerlik göstermekteydi. Sonuç: Bulbar üretral darlıklarda; substitüsyon üretroplastide dorsal ve ventral onlay greft yöntemleri yüksek başarı ve güvenle tercih edilebilir. Anahtar Kelimeler: Dorsal Greft; Substitüsyon Üretroplasti; Üretral Darlık; Ventral Greft ABSTRACT Objective: Urethral stenosis is most commonly observed in the bulbar urethra in males. We aimed to investigate the surgical effects of the graft site in substitution urethroplasty with buccal mucosal graft implantation in stenosis longer than 2 cm in the bulber urethra. Material and Methods: Demographic and stenosis characteristics of patients who had underwent dorsal and ventral graft onlay urethroplasty for bulbar urethral stenosis longer than 2 cm were evaluated retrospectively. The success and complication rates of the dorsal and ventral graft urethroplasty groups were compared. The absence of voiding complaints and the maximum flow rate above 15 ml / s were accepted as success criteria. Results: Urethroplasty was performed in 52 patients (83.9%) with dorsal onlay grafts and 13 patients (81.2%) with ventral onlay grafts. There was no significant difference between the groups. The two groups were similar in terms of demographic and stenosis characteristics, operative and surgical outcomes, and complication rates (14.5% and 12.5%). Conclusion: In bulbar urethral stenosis, substitution urethroplasty, dorsal and ventral onlay graft methods can be preferred with high success and confidence.
