Öz

ÖZET

Akut apandisit acil cerrahi gerektiren akut karın nedenlerinin başlıca nedenidir. Tanı ve tedavide gecikme

önemli komplikasyonlara yol açabilir. Serum inflamasyon belirteçleri anammez, fizik muayene ile birlikte yaygın

olarak başvurulan ucuz ve ulaşılabilir tanısal araçlardır. C-reaktif protein (CRP) inflamasyonda karaciğerden

en hızlı, en çok sentezlenen akut faz reaktanı olup akut apandisitde tanısal amaçlı kullanılan inflamasyon

belirteçidir. Literatürde preoperetif ve postoperatif CRP değerlerinin perfore olmayan akut apandisitlerin histopatolojik

alt gruplarına göre karşılaştıran kısıtlı çalışma var. Bu amaçla Eylül 2008- Aralık 2008 tarihleri arasında

T.C.S.B. İzmir Tepecik Eğitim ve Araştırma Hastanesi Genel Cerrahi Kliniğinde akut apandisit nedeniyle

appendektomi uygulanan hastaların ameliyat öncesi ve sonrası CRP değerlerinin histopatolojik alt gruplarla

karşılaştırılması yapıldı. Perfore ve non-perfore flegmenöz alt grup apandisitlerde postoperatif CRP değerlerinin,

preoperatif değerlere göre anlamlı derecede yükseldiği (p<0,005, p<0,001), inflamasyonun daha az

olduğu eritematöz (p=0,05) ve oblitere akut apandisitlerde (p=0,38) ise anlamlı bir fark olmadığını saptadık.

Non-perfore flegmenöz apandisitlerde inflamasyon şiddetiyle orantılı olarak CRP değerlerinde preoperatif

yükselmeye yol açtığı ve postoperatif süreçte artarak devam etdiği gösterilmiştir. İnflamasyonun şiddetinin

daha az görüldüğü diğer histolojik alt gruplarda tanısal değeri olmasına rağmen postoperatif değerlerde anlamlı

değişiklik saptanmadı.

ABSTRACT

Acute appendicitis is one of the leading causes of acute abdomen. The delay in diagnosis and treatment

can lead to significant complications. Serum inflammation markers are cheap, accessible diagnostic tools

commonly used along with history and physical examination. C-reactive protein (CRP) is the fastest, most

synthesized acute phase reactant in response to inflammation in the liver and is the diagnostic marker for

inflammation in acute appendicitis. There are limited studies comparing preoperative and postoperative CRP

values of histopathological subgroups of non-perforated acute appendicitis in the literature. The preoperative

and postoperative CRP values of patients who underwent appendectomy for acute appendicitis at the T.C.S.B.

İzmir Tepecik Training and Research Hospital General Surgery Clinic were compared with the histopathological

subgroups between September to December in 2008. Postoperative CRP values of perforated and non

perforated phlegmonous subgroup of appendicitis were found to be significantly higher than pre-operative

values (p <0.005, p <0.001). p = 0.38) but no significant difference in erythematous (p = 0.05) and obliterated

acute appendicitis (p = 0.38). It has been shown that non-perforated phlegmoneus appendicitis leads to a

pre-operative increase in CRP values in proportion to the severity of inflammation and continues to increase

in the post-operative process. Despite the diagnostic value of other histological subgroups with less severity

of inflammation, there were no significant changes in postoperative values.
