Endometri̇al Örnekleme Sonuçlarımız: 1149 Olgunun Analizi

Öz
Amaç: Bu çalışmamızda tersiyer bir merkez olan kliniğimizde yapılan endometrial örneklemelerde endikasyonlar ve histopatolojik sonuçlar arasındaki ilişkiyi araştırmayı amaçladık.
Gereç ve Yöntem: Sakarya Eğitim ve Araştırma Hastanesinde Ocak 2015- Mart 2021 tarihleri arasında jinekolojik nedenler ile endometrial örnekleme yapılan 1149 hastanın patoloji sonuçları retrospektif olarak değerlendirildi. Postmenopozal endometrial kalınlık artışı için sınır değer 5 mm olarak kabul edildi.
Bulgular: Çalışmaya dahil olan hastaların yaş ortalaması 47,58 idi. 813(%70,8) tanesi premenopozal dönemdeyken, 336(%29,2) tanesi postmenopozal dönemdeydi. Endometrial örnekleme için en sık endikasyonun anormal uterin kanama olduğu izlendi. Endometrium kanseri için ise en riskli grubu postmenopozal kanaması olan hastalar oluşturduğu izlendi.
Sonuç: Endometrium kanseri riskinin arttığı anormal uterin kanama, postmenepozal kanama, postmenepozal endometrial kalınlık artışı olan ve tamoksifen kullanan hastalarda endometrial örnekleme yapılması uygun bir yaklaşım olacaktır. Bunula beraber beningn nedenlerle histerektomi yapılcak hastalara preopretif rutin örnekleme yapılmasını destekler sonuçlara ulaşamadık.

Aim: In this study, we aimed to investigate the relationship between the indications and histopathological results in endometrial sampling performed in our clinic, which is a tertiary center.
Material and Method: Pathology results of 1149 patients who underwent endometrial sampling for gynecological reasons at Sakarya Training and Research Hospital between January 2015 and March 2021 were evaluated retrospectively. The cut-off value for postmenopausal endometrial thickness increase was accepted as 5 mm.
Results: The mean age of the patients was 47.58. 813(70.8%) of them were in the premenopausal period and 336 (29.2%) of them were in the postmenopausal period. The most common indication for endometrial sampling was abnormal uterine bleeding. It was observed that patients with postmenopausal bleeding were the most at-risk group for endometrial cancer.
Conclusion: Endometrial sampling would be an appropriate approach in patients with abnormal uterine bleeding, postmenopausal bleeding, increased postmenopausal endometrial thickness, and those using tamoxifen, which are at increased risk of endometrial cancer. However, we could not find results that support routine preoperative sampling in patients undergoing hysterectomy for benign reasons.
