The Effectiveness of Group and Individual+Group Schema Therapy on Borderline Personality Disorder: A Systematic Review

Öz

Borderline Personality Disorder (BPD) is a complex mental disorder that profoundly impacts individuals' emotional stability, interpersonal relationships, and self-regulation skills. In recent years, Group Schema Therapy (GST) and Individual + Group Schema Therapy (IGST) have received increasing attention in the treatment of BPD. The current study aims to systematically review empirically based research investigating the effects of GST and IGST on BPD. The review was prepared based on the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) criteria and articles published after 2009 were searched from databases such as PubMed, ScienceDirect / Elsevier, Wiley Online Library, and PsycINFO. Additionally, some articles were manually found and included, and a total of eight studies were reviewed in terms of their therapeutic impacts. The studies were analyzed based on types of therapy, participant profiles, techniques, and treatment outcomes. The findings revealed that both approaches are effective in reducing the severity of BPD symptoms, enhancing quality of life, and improving overall functioning of individuals. IGST was found to be particularly superior in reducing suicidal ideation and alleviating symptom severity. Group therapies were noted to accelerate the recovery process through fostering a sense of social belonging and emotional sharing, while individual therapies were found to be more important for in-depth therapeutic work. In conclusion, GST and BGST appear to be promising approaches for the treatment of BPD. However, factors such as therapy duration, intensity, and the number of therapists may affect treatment outcomes. Therefore, further comprehensive research is needed in this field.

Borderline Kişilik Bozukluğunun Tedavisinde Grup ve Bireysel + Grup Şema Terapisinin Etkililiği: Sistematik Bir Gözden Geçirme

Öz

Borderline Kişilik Bozukluğu (BKB), bireylerin duygusal istikrarını, kişilerarası ilişkilerini ve öz düzenleme becerilerini derinden etkileyen karmaşık bir psikolojik bozukluktur. BKB'nin tedavisinde, grup şema terapisi (GŞT) ve bireysel + grup şema terapisi (BGŞT) son yıllarda giderek daha fazla ilgi görmektedir. Mevcut çalışmanın amacı, GŞT ve BGŞT’nin BKB üzerindeki etkilerini inceleyen görgül araştırmaları sistematik bir şekilde değerlendirmektir. Derleme, Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) kriterleri esas alınarak hazırlanmış, “PubMed”, “ScienceDirect / Elsevier”, “Wiley Online Library” ve “PsycINFO” gibi veri tabanlarından 2009 sonrası yayımlanmış makaleler taranmıştır. Buna ek olarak, bazı makaleler elle bulunarak dahil edilmiş ve toplamda 8 çalışma terapötik etkiler açısından değerlendirilmiştir. Çalışmalar, terapi türleri, katılımcı profilleri, uygulama teknikleri ve tedavi sonuçları çerçevesinde ele alınmıştır. Bulgular, her iki yaklaşımın da BKB semptomlarının şiddetini azaltmada, yaşam kalitesini artırmada ve bireylerin genel işlevselliğini iyileştirmede etkili olduğunu ortaya koymuştur. BGŞT, özellikle intihar düşüncelerini azaltma ve semptom şiddetini hafifletme açısından daha üstün bulunmuştur. Grup terapilerinin sosyal aidiyet hissi ve duygusal paylaşım gibi unsurlarla bireylerin iyileşme sürecini hızlandırdığı; bireysel terapilerin ise daha derinlemesine çalışmalar için önemli olduğu saptanmıştır. Sonuç olarak, GŞT ve BGŞT’nin BKB tedavisinde umut vadeden yaklaşımlar olduğu görülmektedir. Bununla birlikte, tedavi süresi, yoğunluğu ve terapist sayısı gibi faktörlerin tedavi sonuçlarını etkileyebileceği ve bu alanda daha kapsamlı çalışmalara ihtiyaç duyulduğu söylenebilir.
