Maksiller Sinüs Patolojilerinin KIBT ile Değerlendirilmesinde Uzmanlık Öğrencileri Arasındaki Uyum

Öz

ÖZET Giriş Maksiller sinüs patolojileri diş hekimliği pratiğinde sıklıkla karşılaşılan rastlantısal bulgulardır. Konik ışınlı bilgisayarlı tomografi, paranazal sinüslerin değerlendirilmesinde düşük doz radyasyon ve yüksek kemik çözünürlüğü avantajları nedeniyle giderek daha fazla tercih edilmektedir. Bu çalışmanın amacı, maksiller sinüs patolojilerini konik ışınlı bilgisayarlı tomografi ile değerlendirmede ağız, diş ve çene radyolojisi uzmanlık öğrencileri arasındaki gözlemciler arası uyumu analiz etmektir. Gereç ve Yöntem Helsinki Deklarasyonu ilkelerine uygun olarak yürütülen ve Klinik Araştırmalar Etik Kurulu tarafından onaylanan bu retrospektif gözlemsel çalışmada, 18 yaş üzerindeki 70 hastaya ait toplam 140 maksiller sinüs değerlendirildi. Travma geçmişi, cerrahi öyküsü veya tümöral oluşumu bulunan hastalar çalışma dışı bırakıldı. Bir ve iki yıl deneyime sahip iki radyoloji asistanı konik ışınlı bilgisayarlı tomografi görüntülerini bağımsız olarak değerlendirdi. 1 mm kalınlığındaki cross-sectional ve tangential kesitler incelendi. Gözlemciler arası uyum Kappa katsayısı ile hesaplandı ve Landis ve Koch sınıflandırmasına göre yorumlandı. Bulgular Her iki gözlemcide de en sık gözlenen patoloji mukozit olup, sırasıyla %59,6 ve %58,2 oranında saptandı; bunu %22,7 ve %21,5 oranlarıyla sinüzit izledi. Mukozit, mukus retansiyon kisti, polip ve sinüzit için gözlemciler arası uyum 0,94 ile 1,00 arasında değişen kappa değerleriyle mükemmel düzeyde bulundu. Ancak periostitis için kappa katsayısı 0,38 olarak hesaplandı; bu durum sınırlı vaka sayısı ve patolojinin değişken radyolojik görünümüne bağlanabilir. Genel gözlemciler arası uyum 0,940 kappa değeri ile mükemmel düzeyde bulundu. Sonuç Bu çalışma, konik ışınlı bilgisayarlı tomografi kullanılarak belirgin morfolojik özelliklere sahip maksiller sinüs patolojilerinin değerlendirilmesinde uzmanlık öğrencileri arasında mükemmel düzeyde uyum olduğunu göstermiştir. Bulgular, radyoloji asistanlarının yaygın maksiller sinüs patolojilerini güvenilir şekilde tanımlayabildiğini ortaya koymaktadır. Daha geniş örneklem büyüklüğü ve farklı deneyim düzeylerindeki radyologların katılımıyla yapılacak gelecek çalışmalar, bu bulguların genellenebilirliğini arttıracaktır.

Inter-Observer Agreement Among Residents in the Evaluation of Maxillary Sinus Pathologies with CBCT

ABSTRACT Introduction Maxillary sinus pathologies are frequently encountered incidental findings in dental practice. Cone-beam computed tomography is increasingly preferred in the evaluation of paranasal sinuses due to its advantages of low-dose radiation and high bone resolution. The aim of this study is to analyze the inter-observer agreement among oral and maxillofacial radiology residents in evaluating maxillary sinus pathologies with cone-beam computed tomography. Materials and Methods In this retrospective observational study, conducted in accordance with the principles of the Declaration of Helsinki and approved by the Clinical Research Ethics Committee, a total of 140 maxillary sinuses from 70 patients over 18 years of age were evaluated. Patients with a history of trauma, surgical history, or tumoral formations were excluded from the study. Two radiology residents with one and two years of experience independently evaluated the cone-beam computed tomography images. Cross-sectional and tangential sections with 1 mm thickness were examined. Inter-observer agreement was calculated using the Kappa coefficient and interpreted according to the Landis and Koch classification. Results The most frequently observed pathology in both observers was mucositis, detected at rates of 59.6% and 58.2%, respectively, followed by sinusitis at rates of 22.7% and 21.5%. Inter-observer agreement for mucositis, mucus retention cyst, polyp, and sinusitis was found to be excellent with kappa values ranging from 0.94 to 1.00. However, the kappa coefficient for periostitis was calculated as 0.38, which can be attributed to the limited number of cases and the variable radiological appearance of the pathology. Overall inter-observer agreement was found to be excellent with a kappa value of 0.940. Conclusion This study demonstrated excellent agreement among residents in evaluating maxillary sinus pathologies with distinct morphological features using cone-beam computed tomography. The findings reveal that radiology residents can reliably identify common maxillary sinus pathologies. Future studies with larger sample sizes and participation of radiologists with different levels of experience will increase the generalizability of these findings.
