Evde Enteral Tüple Beslenen Hastaların Beslenmeye İlişkin Yaşadığı Sorunlar ve Yakınlarının Bakım Yükü

Öz

Giriş: Günümüzde teknolojinin hızla gelişmesi ile enteral tüple beslenme, hastanın kendi ev ortamında ailesi ile birlikte yaşamını sürdürebilmesine olanak sağlayan bir yöntemdir. Bu hastaların bakım sorumluluğu da hasta yakınlarına düşmektedir. Amaç: Bu çalışma evde enteral tüple beslenen hastaların beslenmeye ilişkin yaşadığı sorunların ve yakınlarının bakım yüklerinin belirlenmesi amacıyla yapılmıştır. Yöntem: Kesitsel ve tanımlayıcı nitelikteki bu araştırmanın örneklemini hastanede enteral tüp yerleştirilen ve evde beslenmesini bu yolla sürdürmeye devam eden, ilk bir ayını tamamlamış olan 49 yetişkin hasta yakını oluşturmuştur. Veriler, Hasta ve Bakım Verici Bilgi Formu, Evde Enteral Tüple Besleme’ye (EETB) yönelik yaşanılan sorunlar formu ve Zarit Bakım Verme Yükü Ölçeği (ZBYÖ) ile toplanmıştır. Veriler Mann- Whitney U testi ve Kruskall Wallis önemlilik testi ile değerlendirilmiştir. Bulgular: Çalışmamızda hasta yakınlarının bakım yükünün orta düzeyde (35.18±12.45) olduğu, günlük yaşam aktivitelerinin sürdürülmesinde başkasına bağımlı hastaların yakınlarının bakım yükünün daha fazla olduğu bulunmuştur. Çalışmamızda gastrostomi bölgesinde enfeksiyon komplikasyonu ile karşılaşan hasta yakınlarının bakım yükünün istatistiksel olarak önemli düzeyde arttığı belirlenmiştir (p<.05). Hastalarda EETB’ye ilişkin en fazla kilo kaybı (%59.2), konstipasyon(%53.1), bulantı-kusma (%49.0) ve diyarenin (%14.3) geliştiği, bu komplikasyonlara yönelik hasta yakınlarının hissettiği bakım yükünün istatistiksel olarak önemli olmasa da daha yüksek olduğu saptanmıştır (p>.05). Sonuç: Hasta yakınlarının EETB ilişkin minör sorunlarla karşılaştıkları, bununla birlikte hastasında sorun gelişen hasta yakınlarının bakım yükünün daha yüksek olduğu söylenebilir.Anahtar Kelimeler: Evde Enteral Beslenme, Hasta Yakını, Bakım Yükü.

Nutrition Problems Faced by Patients Fed with the Enteral Tube at Home and The Burden of Care on Their Relatives

Öz

Background: The rapid development of technology today with enteral tube feeding is a method that enables the patient can continue to live with his family in their home environments. Responsibility for the care of these patients falls on patients’ relatives. Objectives: The study was conducted to determine nutrition problems of patients fed with the enteral tube at home and the burden of care on their relatives. Methods: The sample of this cross-sectional, descriptive study comprised 49 adult patients to whom an enteral tube was inserted at the hospital and who were fed via the tube for the last month. The study data were collected using the Patient and Caregiver Information Form, Home Enteral Nutrition-Related Problems (HENRP), “Zarit Caregiver Burden Scale” for the data analysis, the Mann-Whitney U test and Kruskal-Wallis test were used. Results: In the present study, the caregiver burden of the relatives of the patients was moderate (35.18 ± 12:45). Of the patient relatives, those who gave care to patients dependent on someone else while performing activities of daily living felt the burden of care more. In the present study, the caregiver burden of the relatives of the patients who had infectious complication was determined to increase statistically significantly (p <.05). Other most common HENRP-related complications the patients developed were weight loss (59.2%), constipation(53.1%), nausea and vomiting(49.0%) and diarrhea(14.3%). The caregiver burden of the relatives of the patients who experienced these complications, although statistically insignificant, was higher (p> .05). Conclusion: Although the relatives of the patients can be said to observe HENRP-related minor problems or complications in patients to whom they gave care, of them, those who gave care to patients having problems had a higher burden of care
