Examination of the Effect of Music Upon the Postoperative Pain Management in Pediatric Patients Who Are Hospitalized in the Orthopedics Service

Background: Children who have had orthopedic surgery are suffer from severe pain. In order to cope with this pain, music is a recommended method. A study which researches the effect of music upon the postoperative pain in pediatric patients could not be reached in Turkey. Objectives: The aim of this study was to examine the effects of music upon the postoperative pain in pediatric patients in the age group of 7-12, who had undergone an orthopedic surgery. Methods: The research was designed as a semi-experimental. The study was conducted between June and October 2011 at the orthopedic and traumatology service of a university hospital. The sample of the study consisted of 60 children aged 7-12 years. The classical music was listened to the children who were in experimental group after the surgery. Description form of children who had undergone an orthopedic surgery and their families, which was prepared by the researcher, and Visual Analog Scale was used for the collection of the data. As well as the percentage and chi-square tests, the significance test (t test) was used for the difference between two averages in independent groups for the analysing the data. Results: There is a similarity between the experimental and control group, in terms of the distribution of variables regarding their age, gender, type of surgery, whether the patient has had previous surgery, the first postoperative analgesic time and type of analgesic drug, and the difference between two groups was found to be statistically insignificant (p > .05). According to the results obtained from the study, it was determined that the pain scores of children in the experimental group who had undergone a surgery were significantly lower within the first 6 hours compared to the children in the control group (p < .05), there was no statistical difference between the pain scores of pediatric patients in the experimental and control groups within the first 12 and 24 hours (p > .05). Conclusion: Music was suggested to be included in nursing practices as an non-pharmacological nursing intervention, due to its positive effect within the first 6 hours during the postoperative period.

Ortopedi Servisinde Yatan Çocuk Hastalarda Ameliyat Sonrası Ağrı Yönetiminde Müziğin Etkisinin İncelenmesi

Öz

Giriş: Ortopedi ameliyatı olmuş çocuk hastalar ağrıyı şiddetli yaşamaktadır. Bu ağrıyla baş etmede müzik önerilen yöntemlerden biridir. Türkiye’de, çocuk hastalarda ameliyat sonrası ağrı üzerinde müziğin etkisini inceleyen bir çalışmaya ulaşılamamıştır. Amaç: Ortopedi ameliyatı olmuş 7-12 yaş arası çocuk hastalarda müziğin ameliyat sonrası ağrı üzerindeki etkilerini incelemektir. Yöntem: Araştırma yarı deneysel olarak tasarlandı. Araştırma Haziran-Ekim 2011 tarihleri arasında bir üniversitesi hastanesinin Ortopedi ve Travmatoloji Servisi’nde yürütüldü. Araştırmanın örneklemini, ameliyat olmuş 7-12 yaş grubundaki 60 çocuk hasta oluşturdu. Girişim grubundaki çocuklara ameliyat sonrası klasik müzik dinletildi. Verilerin toplanmasında çocuk hastaların ve ailelerinin tanıtım formu, Vizüel Analog Skala (VAS) kullanıldı. Verilerin analizinde, yüzdelik ki-kare ve bağımsız gruplarda iki ortalama arasındaki farkın önemlilik testi kullanıldı. Bulgular: Girişim ve kontrol grupları yaş, cinsiyet, ameliyat türü, ameliyat deneyimi, ameliyat sonrası uygulanan ilk analjezik zamanı ve türü ile ilgili değişkenlerin dağılımları açısından birbirine benzer bulundu (p > .05). Girişim grubundaki çocukların ilk 6 saatteki ağrı puanlarının, kontrol grubundaki çocuklara göre anlamlı düzeyde daha düşük olduğu (p < .05), ilk 12 ve 24 saatte ise girişim ve kontrol grubundaki çocukların ağrı puanları arasında istatistiksel açıdan anlamlı bir farklılık olmadığı belirlendi (p > .05). Sonuç: Müziğin ameliyat sonrası ilk 6 saatte olumlu etkisi nedeniyle, farmakolojik olmayan hemşirelik girişimi olarak hemşirelik uygulamalarına dahil edilmesi önerilmiştir.
