Evaluation of intoxication cases admitted to Emergency Department of a University Hospital

Objectives: This study is aimed at the evaluation of the demographic properties of the patients from Sivas and sur-rounding cities, presenting to the emergency department by poisoning. Materials and methods: Our study was carried out on 567 patients admitted to Cumhuriyet University Faculty of Medicine\'s Emergency Department in four years because of poisoning. Patients were divided into groups such as su-icides, accidentals, or being exposed. All the patients were defined about their ages, sex, participation of the sea-sons, consciousness, prognosis, exposure to the type of poison and what had been done before arriving to us. Mush-room poisonings were distinguished into two groups as early or later symptomatic. Results: Suicide patients were 363 (64.10%), accidentals were 120 (21.28 %), exposed patients were 83 (14.62 %). Amaç: Bu çalışmada, Sivas ve çevre illerden acil servise zehirlenme nedeniyle başvuran hastaların demografik özel-liklerinin değerlendirilmesi amaçlanmıştır. Gereç ve yöntem: Çalışmamıza Cumhuriyet Üniversitesi Tıp Fakültesi Acil Tıp Anabilim Dalında dört yıl içerisinde zehirlenme nedeniyle tedavi edilen 567 hasta alındı. Hastalar zehirlenme sebeplerine göre intihar, kaza, maruz kalma şeklinde guruplara ayrıldı. Zehirlenmeler genel ve guruplara ayrılmış olarak yaş, cinsiyet, bilinç durumu, başvurdukları mevsim, zehirlenme şekli, prognozları, ne ile zehirlendikleri ve önceki kliniklerde yapılan müdahaleler açısından ince-lendiler. Mantar zehirlenmesi erken ve geç bulgu verenler olarak sınıflandırıldı. Bulgular: Hastaların 363\'ü (%64,10) intihar, 120\'si (%21,28) kaza, 83\'ü (%14,62) maruz kalmaydı. Yaş sınırları 15-81, yaş ortalaması 27,82±11,56 bulundu. Bütün zehirlenme vakaların 172\'si (%30,3) erkek ve yaş ortalamaları 29,97±5,122 iken 395\'i (%69,7) kadın ve yaş ortalamaları 26,89±2,072 olarak bulundu. Hastaların geldiklerinde bilinç-leri; %88,2\'sinde açık, %7,6\'sinde konfüze, %4,1\'inde kapalıydı. Biri ise (%0,2) ölü idi. En fazla ilaç zehirlenmesi (%72,7) sonra mantar (%12,2) ve tarım ilaçları (%9) ile zehirlenmeler görüldü. Zehirlenmelerin tamamı içinde 2 hasta organofosfat, 3 hasta metanol, 3 hasta ilaç zehirlenmesi, bir hasta mantar ve bir hasta tüp gazı zehirlenmesiyle öldü. Zehirleyen maddeye göre dağılım incelendiğinde ise en fazla çoklu ilaç (%27) görüldü. Mantar zehirlenmelerinde sık-lıkla semptomların erken (0-6 saat) başladığı tespit edilmiş iken (%88,5), geç (6-24 saat) semptom görülenler daha az olarak bulundu (% 11,5). En fazla zehirlenme kış (%30) mevsiminde görüldü. Önceki kliniklerde yapılan müdahaleler incelendiğinde ise, hiçbir müdahale yapılmayanlar (%52,4) en fazla görüldü. Sonuç: Ülkemizde zehirlenme ön tanısıyla sağlık kuruluşlarına başvuran hastalar hakkında önemli sayıda istatistiki veriler bulunmakla birlikte bizim çalışmamız Sivas yöresindeki zehirlenmeler konusunda değerli bilgiler vermektedir.
