The Use Of Epithesis For Gingival Recession: A Case Report

Öz

Periodontal problems occurring on the anterior maxillar area may cause aesthetic problems due to prolonged clinical crown length and loss of papillas. This type of epithesis is preferred as a conservative treatment method for the reconstruction of deformities occurs after treatment of periodontal diseases and elimination of periodontal pockets, especially in the anterior maxilla. The aim of this case report is to explain that the aesthetic problems can be restored by using non-invasive procedures like gingival epithesis after correction of periodontal problems and to explain the indications / contraindications, advantages / disadvantages, production stages and materials used in the production stage of gingival epithesis.
A 40-year-old female patient was admitted to our clinic with complaints of halitosis, gingival bleeding and esthetic problems from elongated anterior teeth. In the clinical and radiographic examinations, advanced vertical and horizontal bone loss, mobility, bleeding with probing and gingival recession were observed on the maxillar and mandibuler tooth. The patient was diagnosed as generalized chronic periodontitis and underwent oral hygiene training, calculus scaling, root planing and flap operation. Despite the improvement of the current periodontal condition, aesthetic problems persisted. In order to solve aesthetic problems, it was decided to treat the patient with gingival episthesis.
After 1 month at control appointment it was observed that the patient was satisfied with the current aesthetic condition and periodontal tissues was healty. In conclusion, in this case, aesthetic problems were solved by gingival epithesis as an alternative to advanced periodontal surgical interventions.

Dişeti Çekilmesinde Epitez Kullanımı: Olgu Raporu

Öz

Üst çene ön bölgede meydana gelen periodontal problemler, uzamış klinik kuron boyu ve papil kaybı nedeniyle estetik olmayan bir görünüme sebep olabilirler. İleri seviyedeki dişeti çekilmelerinde uygulanan girişimsel periodontal cerrahilerin sonuçlarının çoğu zaman öngörülememesi, iyileşme süreçlerinin uzun olması ve maliyeti, dişeti epitezlerinin kullanım alanlarını genişletmiştir. Bu tür epitezler özellikle üst çene ön bölgede, periodontal hastalıkların kontrol altına alınmasından ve periodontal ceplerin elimine edilmesinden sonra oluşan deformitelerin düzeltilmesinde konservatif bir tedavi yöntemi olarak tercih edilmektedir. Bu olgu sunumunun amacı periodontal problemlerin girişimsel olmayan işlemlerle düzeltilmesinden sonra mevcut estetik sorunların dişeti epitezi ile restore edilebildiğini göstermek ve dişeti epitezlerinin endikasyon/kontrendikasyon, avantaj/dezavantaj, üretim aşamaları ve üretim aşamasında kullanılan materyalleri açıklamaktır.
40 yaşında kadın hasta ağız kokusu, dişetinde kanama ve uzamış dişlerden kaynaklı estetik şikâyetlerle kliniğimize başvurdu. Hastadan alınan anamnezde herhangi bir sistemik rahatsızlığı olmadığı öğrenilmiştir. Yapılan klinik ve radyografik incelemelerde, alt ve üst çenede ileri seviyede vertikal ve horizontal kemik kayıpları, diş mobiliteleri, sondlama ile kanama ve dişeti çekilmeleri görülmüştür. Generalize kronik periodontitis tanısı konulan hastaya oral hijyen eğitimi, diştaşı temizliği, kök yüzeyi düzleştirme ve flep operasyonu uygulanmıştır. Hastada mevcut periodontal durumun düzelmesine rağmen estetik problemlerin devam ettiği görülmüştür. Estetik problemleri çözmek amacıyla hastaya dişeti epitezi yapılmıştır.
1 ay sonra yapılan kontrolde hastanın mevcut estetik durumdan memnun olduğu ayrıca periodontal dokunun sağlığını koruduğu görüldü. Sonuç olarak bu olguda, estetik problemler ileri periodontal cerrahi girişimlere bir alternatif olarak dişeti epitezi ile sağlanmıştır.
