Periferik İntravenöz Kateter İlişkili Komplikasyonların Bakımında Lokal Sıcak ve Soğuk Uygulamaların Etkisi: Sistematik Derleme

Öz

Amaç: Bu çalışma periferik intravenöz kateter komplikasyonlarında uygulanan lokal sıcak veya soğuk uygulamaların etkinliğini sistematik olarak incelenmek amacıyla yapılmıştır.
Yöntem: Konuyla ilgili 2012 ile 2022 tarihleri arasında yayınlanmış 415 çalışma incelenmiştir. Çalışmada PubMed, Web of Science, Skopus, Science Direct, Ulakbim, Cochrane veri tabanlarında yer alan çalışmalar ve araştırmaya dahil edilme kriterlerine uyan toplam 15 çalışma sistematik olarak incelenmiştir. Tarama yapılırken “sıcak kompres”, “periferik venöz kateter” “komplikasyon”, “lokal sıcak uygulama”, “sıcak uygulama”, “ılık uygulama”, “ılık kompres”, “soğuk uygulama”, “soğuk kompres”, “flebit”, “infiltrasyon”, “ekstravazasyon” anahtar kelimeleri hem İngilizce hem Türkçe farklı kombinasyonlar kullanılmıştır. Çalışmaya 15 araştırma dahil edilmiştir.
Bulgular: Bu çalışmada toplam 15 çalışma uygunluk kriterlerini karşılamıştır. Araştırma kapsamına alınan makalelerin %80’i (n=12) araştırma makalesi, %20’si (n=3) tez çalışmasıdır. Uygulama türü değerlendirildiğinde %20’si (n=3) ılık uygulama, %33.4’ü (n=5) sıcak uygulama, %20’si (n=3) soğuk uygulama ve %26.6’sı (n=4) hem sıcak hem de soğuk uygulamayı içerdiği belirlenmiştir. İncelenen çalışmalarda sıcak ve soğuk uygulamaların periferik venöz katetere bağlı komplikasyonların semptomlarında azalma ve iyileşme sağladığı belirlenmiştir.
Sonuç: Hastalarda flebit, tromboflebit, infiltrasyon ve ekstravazasyon komplikasyonları geliştiği ve bu komplikasyonlara uygulanan sıcak veya soğuk uygulamaların iyileşmede olumlu etkisi olduğu belirlenmiştir. Önlenebilir olan periferik venöz kateter komplikasyonları ile sık karşılaşılmaktadır. Sıcak ve soğuk uygulama girişimlerine ilişkin sınırlı bilgi ve hemşirelik uygulaması yer almaktadır. Bu doğrultuda, periferik venöz kateter ile ilişkili komplikasyonların bakımına yönelik büyük örneklemle kanıt düzeyi yüksek araştırmaların yapılması önerilmektedir.

Effect of Local Hot and Cold Applications on the Care of Peripheral Intravenous Catheter-Related Complications: Systematic Review

Öz

Objective: This study was conducted to systematically examine the effectiveness of local hot or cold applications applied in peripheral intravenous catheter complications.
Methods: 415 studies published between 2012 and 2022 on the subject were examined. In the study, studies in PubMed, Web of Science, Skopus, Science Direct, Ulakbim, Cochrane databases and a total of 15 studies that met the inclusion criteria were systematically examined. While scanning, “warm compress”, “peripheral venous catheter” “complication”, “local hot application”, “hot application”, “warm application”, “warm compress”, “cold application”, “cold compress”, “phlebitis”, “infiltration”, “extravasation” keywords were used in different combinations in both English and Turkish. 15 studies were included in the study.
Results: A total of 15 studies met the eligibility criteria in this study. 80% (n=12) of the articles included in the research are research articles and 20% (n=3) are thesis studies. When the application type is evaluated, 20% (n=3) warm application, 33.4% (n=5) hot application, 20% (n=3) cold application and 26.6% (n=4) both hot application. It was determined that it included both cold application. In the studies reviewed, it was reported that hot and cold applications provided a reduction and improvement in the symptoms of peripheral venous catheter-related complications.
Conclusion: It was determined that phlebitis, thrombophlebitis, infiltration and extravasation complications developed in patients and hot or cold applications applied to these complications had a positive effect on recovery. Peripheral venous catheter complications, which are preventable, are frequently encountered. There is limited information and nursing practice regarding hot and cold application interventions. In this regard, it is recommended to conduct high-evidence studies with a large sample for the care of peripheral venous catheter-related complications.
