How do Turkish people perceive colonoscopy procedure? A Research on the factors effecting adaptation to colonoscopic preliminary diet: A clinical prospective study

Abstract

Background and Aims: The aim of this study is to investigate the factors that have an affect on colon cleaning when nap was used before colonoscopy for intestinal cleanliness and evaluate how our society perceives the colonoscopy prosedure by way of a public survey. Materials and Method: This study was carried out in Ankara University School of Medicine Cebeci Hospital Endoscopy Center between the dates August 2008 and March 2009. The cleanliness degree of colon was scored in colonoscopy procedure. All patients were questioned by the nurse with a survey before and after the prosedure. Results: The average age of the104 patients that have taken the survey was 52.3 (21-82) SD 14.1. There were no age differences between groups of high and low education levels (p=0.410), and (n=59) 56.2% of the patients surveyed were women, (n=45) 42.9% were men. There were no sex differences between groups of high and low education levels. In 6 patients intestinal cleanliness was bad, in 36 patients intestinal cleanliness was acceptable, in 62 patients intestinal cleanliness was good-perfect.The most commonly seen side effects were stomach ache 30% and nausea 32,8%, for both groups. For all patients stomach ache and nausea were mild. There were no meaningful statistical differences (p=0.203) found in adaptation to preliminary diet (p=0.07) and in quality of intestinal cleanliness between low education level patients (n=68) and high education level patients (n=36). Only 8 of 104 patients had taken the colonoscopy procedure for colorectal cancer scanning. Before the procedure , when patients were asked; 14 of the 36 in high education level patients and 45 of 68 in low education level patients said they were embarrassed because of the procedure (p=0.08). Conclusion: There were no connections found between different education levels and adaptation of preliminary colonoscopy diet and intestinal cleanliness before the colonoscopy. We have also concluded that in asymptomatic individuals over 50 years of age in our society do not pay necessary attention to scanning programs and according to the result of the survey evaluation, it is difficult for a considerable number of people to be psychologically comfortable with the colonoscopy procedure.

Türk insanı kolonoskopi işlemine nasıl bakıyor? Kolonoskopi hazırlık rejimine uyumu etkileyen faktörlerin irdelenmesi: Klinik prospektif çalışma

Öz

Amaç: Bu çalışmada amaç kolonoskopi öncesi oral NaP solüsyonu kullanılarak barsak hazırlığı yapılan hastalarda kolon temizliğini etkileyen faktörlerin incelemek ve toplumumuzun kolonoskopi işlemini nasıl algıladığını yapılan anket sorgulaması ile değerlendirmektir. Yöntem: Bu çalışma Ağustos 2008 - Mart 2009 döneminde Ankara Üniversitesi Tıp Fakültesi Cebeci Hastanesi Endoskopi Merkezi'nde gerçekleştirildi. Kolonoskopi işleminde kolonun temizlik ölçüsü skorlandı. İşlem öncesi ve sonrası tüm hastalar kolonoskopi hemşiresi tarafından anket ile sorgulandı. Bulgular: Çalışmaya alınan 104 hastanın yaş ortalaması 52.3 (21-82) SD 14.1 idi. Eğitim seviyesi düşük ve eğitim seviyesi yüksek grup arasında yaş dağılımı açısından fark yoktu (p=0.410) ve çalışmaya alınan hastaların (n=59) %56.2'i kadın ve (n=45) %42.9'ü erkekti. Eğitim seviyesi düşük ve eğitim seviyesi yüksek grup arasında cinsiyet dağılımı açısından fark yoktu (p=0.860). Altı hastada barsak temizliği kötü, 36 hastada barsak temizliği kabul edilebilir, 62 hastada barsak temizliği iyi-mükemmeldi. Her iki grupta da en sık görülen yan etkiler karın ağrısı %30 ve bulantı %32,8 idi. Tüm hastalarda karın ağrısı ve bulantı hafif düzeydeydi. Eğitim seviyesi düşük hastalar (n=68) ile eğitim seviyesi yüksek (n=36) hasta arasında hazırlık rejimine uyum (p=0.07) ve barsak temizliği kalitesi açısından istatistiksel anlamlı fark saptanmadı (p=0,203). Çalışmamızda kolonoskopi işlemi yapılan 104 hastadan sadece 8 hasta rutin kolorektal kanser taraması için başvurmuştu. İşlem öncesi sorgulmada eğitim seviyesi yüksek 36 hastadan 14 hasta işlemden dolayı utandığını, eğitim seviyesi düşük 68 hastadan 45 hasta işlemden dolayı utandığını söyledi (p=0,08). Sonuç: Kolonoskopi hazırlık rejimine uyum ve kolonoskopi öncesi barsak temizliği, eğitim düzeyi ile ilişkisiz bulundu. Toplumumuzda 50 yaş üstü asemptomatik bireylerin tarama programlarına yeterli ilgiyi göstermediğini ve anket sorgulaması sonuçlarına göre kolonoskopi işlemini hatırı sayılır kişinin psikolojik olarak kabullenmesi zor bir işlem olarak gördüğünü saptadık.
