Yaygın Kas İskelet Sistemi Ağrısı olan Hastalarda Vitamin D Düzeyi

Giriş: Vitamin D’nin kemik mineralizasyonunun yanı sıra metabolik ve nöromusküler stabiliteyi sağlayıcı etkileri de bulunmaktadır. Bu çalışmadaki amaç yaygın kas iskelet sistemi ağrısı (YKA) ile başvuran hastalarda vitamin D düzeyini değerlendirmek, eksikliğinde olası etkili faktörleri belirlemekti. Yöntemler: Çalışmaya Fizik Tedavi ve Rehabilitasyon polikliniğine Ocak 2018 ile Ocak 2020 tarihleri arasında başvuran, kronik bir hastalığı ve sürekli kullandığı bir ilacı olmayan, vitamin D düzeyleri ile kalsiyum, fosfor, alkalen fosfataz ve parathormon takipleri bulunan 241 YKA olan hasta dahil edildi. Hastaların eğitim düzeyi, maddi geliri ve giyim şekli sorgulandı. Giyim şekillerine göre kadın hastalar tüm vücudu güneşe maruz kalabilenler (açık), sadece elleri ve yüzleri güneş görenler (kapalı) olarak ikiye ayrıldı. Bulgular: Hastaların %87.1’i (210) kadın olup, yaş ortalaması 48±10.7 yıl ve vitamin D düzeyi ortalaması 11.3±6.1 ng/ml idi. Kadın hastalarda erkek hastalara göre (p=0.000) ve kapalı hastalarda açık hastalara göre (p=0.000) vitamin D eksikliğinin daha fazla olduğu görüldü. Mevsimlere göre karşılaştırma yapıldığında istatistiksel olarak anlamlı fark olduğu (p=0.003), bu durumun özellikle yaz mevsimi ile ilkbahar mevsimi arasındaki farktan kaynaklandığı belirlendi. Sonuç: YKA olan, özellikle kadın ve kapalı giyim tarzı olan hastalarda ve kıştan ilkbahar aylarına geçişlerde Vitamin D eksikliği dikkate alınmalıdır.

Vitamin D Levels in Patients with Diffuse Musculoskeletal Pain

Introduction: In addition to vitamin D's effects on bone mineralization, it also has effects on metabolic and neuromuscular stability. The aims of this study were to evaluate vitamin D levels in patients with common muscle skeletal system pain (CMP) and to determine possible effective factors in Vitamin D deficiency. Methods: The study included 241 patients with CMP who were admitted to physical therapy and rehabilitation outpatient clinic between January 2018 - January 2020, without any chronic disease or medication use continuously, and who had vitamin D, Ca, P, alkaline phosphatase, and PTH follow-up. The education level, financial income and clothing of the patients were questioned. According to their clothing, women were divided into two groups: those whose body was exposed to the sun (western clothing group), those whose hands and faces were exposed to the sun (conservative clothing group). Results: Most of the patients (%87.1) were female, the mean age of the patients was 48±10.7 years and the mean vitamin D level was 11.3±6.1 ng/ml. Vitamin D deficiency was found to be more prevalent in female patients than in male patients (p=0.000) and in conservative clothing patients than in western clothing patients (p= 0.000). It was found that there was a statistically significant difference when compared with the seasons (p=0.003), which was mainly due to the difference between the summer and the spring season. Conclusion: Vitamin D levels were found to be considerably lower in all of the patients who applied with CMP, in particular vitamin D deficiency should be taken into account in women and in conseravtive clothing patients, and in the transition from winter to spring months.
