Üst hava yolu hacmi, maksiller morfoloji ve osteomeatal kompleks ilişkilerinin konik ışınlı bilgisayarlı tomografi ile değerlendirilmesi

Öz

Amaç: Üst hava yolu hacminin, maksiller morfolojiye ve osteomeatal komplekse ait yapıların birbirleri ile ilişkilerinin değerlendirilmesidir.
Gereç ve Yöntem: 18-35 yaşlar arasında bulunan 83’ü erkek ve 117’si kadın toplam 200 genç erişkin hastadan alınan Konik Işınlı Bilgisayarlı Tomografi (KIBT) görüntüleri retrospektif olarak değerlendirildi. ITK-Snap yazılım programı kullanılarak ölçümler yapıldı. Üst hava yolu hacmi, maksiller iskeletsel genişlik, maksiller dental genişlik, palatal derinlik, nazal kavite genişliği, nazal kavite taban genişliği, apertura piriformis açısı ölçüldü. Bu parametrelerin yaş, cinsiyet, iskeletsel maloklüzyon ile ilişkileri değerlendirildi.
Bulgular: Erkeklerde, üst hava yolu hacmi, maksiller iskeletsel genişlik, maksiller dental genişlik, palatal derinlik, nazal kavite ve nazal kavite taban genişliği daha yüksek iken, apertura piriformisin maksiller açısı istatistiksel olarak daha düşük idi. Sınıf I iskeletsel maloklüzyona sahip hastalarda, maksiller iskeletsel genişlik, apertura piriformisin maksiller açısı Sınıf III iskeletsel maloklüzyona sahip hastalara göre daha yüksek; palatal derinlik istatistiksel olarak daha düşük idi. Hasta yaşı ile nazal kavite (p=0.004) ve nazal kavite taban genişliği (p=0.001) pozitif yönlü, maksiller iskeletsel ark genişliği (p=0.011) negatif yönlü anlamlı korelasyon gösterdi. Üst hava yolu hacmi ile maksiller iskeletsel ark genişliği (p<0.001), palatal derinlik (p<0.001) ve nazal kavite genişliği (p=0.023) arasında pozitif yönlü ve apertura piriformisin maksiller açısı (p=0.005) arasında negatif yönlü istatistiksel olarak anlamlı korelasyon belirlendi.
Sonuç: Genç yetişkinlerde, üst hava yolu hacmi, maksiller morfoloji ve osteomeatal komplekse ait yapıların boyutları birbirleri ile ilişkili olabilmektedir.

Evaluation of upper airway volume, maxillary morphology and osteomeatal complex relationships with Cone-Beam Computed Tomography

Öz

Objective: To evaluate the relationships between upper airway volume, maxillary morphology and structures of the osteomeatal complex.
Materials and Method: Cone-Beam Computed Tomography (CBCT) images taken from a total of 200 young adult patients, 83 male, 117 female, aged between 18-35, were evaluated retrospectively. Measurements were made using the ITK-Snap software program. Upper airway volume, maxillary skeletal width, maxillary dental width, palatal depth, nasal cavity width, nasal cavity floor width, and apertura piriformis angle were measured. The relationships of these parameters with age, gender, and skeletal malocclusion were evaluated.
Results: In males, the upper airway volume, maxillary skeletal width, maxillary dental width, palatal depth, nasal cavity and nasal cavity floor width were higher, while the maxillary angle of the apertura piriformis was lower. In patients with Class I skeletal malocclusion, the maxillary skeletal width and the maxillary angle of the apertura piriformis were higher than in patients with Class III skeletal malocclusion; palatal depth was lower. There was a significant positive correlation between patient age and nasal cavity and nasal cavity floor width (p=0.001), while maxillary skeletal arch width (p=0.011) showed negative significant correlation. A statistically significant positive correlation was determined between upper airway volume and maxillary skeletal arch width(p<0.001), palatal depth (p<0.001) and nasal cavity width (p=0.023), and negative correlation was determined between the maxillary angle of the apertura piriformis (p=0.005).
Conclusion: The volume of the upper airway, the dimensions of the structures of maxilla and osteomeatal complex may be related to each other in young adults.
