Editöre Mektup: Hastanede yatan adölesan COVID-19 hastalarında sistemik immün-inflamatuvar indeksin prognostik değeri

Öz

Çağlar ve arkadaşları tarafından Genel Tıp Derginizde yayımlanan (Cilt 35/Sayı 1, Şubat, ss. 123-128) çalışma, ergen COVID-19 hastalarında Sistemik İmmün-İnflamatuvar İndeks'in (SII) prognostik rolünü ve yoğun bakım ünitesi (YBÜ) kabul riskini diğer biyobelirteçlerle karşılaştırarak incelemektedir. Çalışma, SII'nin basit ve maliyet-etkin bir biyobelirteç olarak YBÜ kabul riskini başarıyla öngördüğünü (AUC: 0.768, p <0.001) göstererek ergen popülasyonda klinik önemine dair değerli veriler sunmaktadır. Ancak, YBÜ'ye kabul edilen hasta sayısının düşük olması (n=14) ve retrospektif tasarım, bulguların genellenebilirliğini sınırlandırmaktadır. Obezite, başvuru zamanı ve SARS-CoV-2 varyantları gibi prognozu etkileyebilecek faktörlerin analiz edilememesi de bir eksikliktir. Gelecekte çok merkezli, prospektif çalışmalar ve longitudinal verilerle SII'nin dinamik değişimlerinin incelenmesi, bu indeksin klinik kullanım potansiyelini daha da netleştirecektir. Çalışma, SII'nin yalnızca COVID-19'da değil, akut kolesistit ve testis torsiyonu gibi diğer durumlarda da prognostik değerini ortaya koyarak, ergen hastalarda erken risk stratifikasyonu ve kaynak optimizasyonu açısından önemli bir katkı sağlamaktadır.

Letter to Editor for Prognostic Value of Systemic Immune-Inflammatory Index in Hospitalized Adolescent COVID-19 Patients

Öz

The study by Çağlar et al., published in your General Medicine Journal (Volume 35/Issue 1, February, pp. 123-128), explores the prognostic role of the Systemic Immune-Inflammation Index (SII) in adolescent COVID-19 patients and its effectiveness in predicting intensive care unit (ICU) admission risk compared to other biomarkers. While the study provides valuable insights into the clinical significance of SII in adolescents, it also has methodological and interpretative limitations. The research highlights SII's potential as a simple, cost-effective biomarker for predicting ICU admission risk (AUC: 0.768, p <0.001), with its calculation based on neutrophil, lymphocyte, and platelet counts aligning well with routine clinical practices. However, the small sample size of ICU-admitted patients (n=14) and the retrospective design limit the generalizability of the findings. Factors such as obesity, time of admission, and SARS-CoV-2 variants were not analyzed, which could influence prognosis. Future multicenter, prospective studies with larger sample sizes and longitudinal data are needed to validate these results and explore SII's dynamic changes during treatment. Despite these limitations, the study makes a significant contribution by demonstrating SII's prognostic value, not only in COVID-19 but also in other conditions like acute cholecystitis and testicular torsion, suggesting its broader clinical utility for early risk stratification and resource optimization in adolescent patients.
