Hand Mental Rotation Task In Patients With Multiple Sclerosis

Abstract

Introduction: Mental rotation is a cognitive function in which objects, images, and the body are mentally rotated. Mental rotation is assessed with the help of various tasks. One of them is hand mental rotation task. Influence on hand mental rotation task in individuals with multiple sclerosis has been shown in limited studies in the literature with the help of computer-based programs. Material-Method: This study was carried out in the Neurology Department of İnönü University Turgut Özal Medical Center. It was held between December 2021 and September 2022. A mobile application called “Orientate” was used to evaluate the hand mental rotation task performance of the patients. A total of 25 hand pictures at different angles were shown and the participants were asked to guess the right/left hand pictures. The number of correct and incorrect in this application and the time to complete the task were recorded. The ratio of correct answers to the total number of questions was calculated. Differences between the two groups were compared using the Independent Sample t test (for normally distributed variables) and the Mann-Whitney U test (for non-normally distributed variables). Categorical variables were evaluated with the Chi-Square Test. Pearson Correlation test was used for the relationship between EDSS scores and mental rotation task. Results: A total of 76 participants were analyzed, including 43 participants in the MS group and 33 participants in the control group. Individuals in the groups were similar in terms of demographic characteristics (age, gender, educational status) (p>0.05). The correct answer rates of the participants in the MS group were significantly lower than those in the control group (p=0.05). There was no significant difference between groups in task completion times (p=0.46). There was a good negative correlation (r=-0.46; p=0.00) between the EDSS score and the accuracy of the mental rotation task, and a very good positive correlation with time (r=0.62; p=00). Conclusion: The results of mental rotation assessment with the help of mobile application in individuals with relapsing-remitting MS showed that accuracy rates decreased and reaction times did not change. It is thought that the absence of a statistical difference between the MS and control groups may be due to the fact that the control group consists of the partners of the MS group and may be due to individual differences.

Multiple Sklerozlu Hastalarda El Mental Rotasyon Görevi

Öz

Giriş-Amaç: Mental rotasyon nesnelerin, görüntülerin ve vücudun zihinsel olarak döndürüldüğü bilişsel bir işlevdir. Mental rotasyon çeşitli görevler yardımıyla değerlendirilir. Bunlardan biri de el mental rotasyon görevidir. Multiple sklerozlu bireylerde el mental rotasyon görevindeki etkilenim bilgisayar tabanlı programlar yardımıyla literatürdeki sınırlı çalışmalarda gösterilmiştir. Gereç-Yöntem: Bu çalışma İnönü Üniversitesi Turgut Özal Tıp Merkezi Nöroloji departmanında yapılmıştır. Aralık 2021 ve Eylül 2022 tarihleri arasında gerçekleştirilmiştir. Hastaların el mental rotasyon görev perfomansını değerlendirmek için “Orientate” adlı mobil uygulama kullanılmıştır. Toplamda 25 adet farklı açılardaki el resimleri gösterilmiş ve katılımcıların sağ/sol el resimlerini tahmin etmeleri istenmiştir. Bu uygulamadaki doğru ve yanlış sayısı ile görevi tamamlama süresi kaydedildi. Doğru cevapların toplam soru sayısına oranı hesaplandı. İki grup arasındaki farklar Independent Sample t testi (normal dağılım gösteren değişkenler için) ve Mann-Whitney U testi (normal dağılım göstermeyen değişkenler için) kullanılarak karşılaştırıldı. Kategorik değişkenler Ki-Kare Testi ile değerlendirildi. EDSS skorları ile mental rotasyon görevi arasındaki ilişki için Pearson Korelasyon testi kullanıldı. Bulgular: MS grubunda 43 katılımcı ve kontrol grubunda 33 katılımcı olmak üzere toplam 76 katılımcı analiz edilmiştir. Gruplardaki bireyler demografik özellikler (yaş, cinsiyet) açısından benzerdi (p>0.05). MS grubundaki katılımcıların doğru cevap oranları kontrol grubundaki katılımcılardan anlamlı olarak daha düşüktü (p=0.05). Görev tamamlama sürelerinde gruplar arasında anlamlı fark yoktu (p=0.46). EDSS skoru ile mental rotasyon görevindeki doğruluk oranı arasında negatif yönde iyi derecede (r=-0.46; p=0.00) ve süre ile pozitif yönde çok iyi (r=0.62; p=00) korelasyon mevcuttu. Sonuç: Relapsing-remitting MS’li bireylerde mobil uygulama yardımıyla mental rotasyon değerlendirmesinin sonuçları doğruluk oranlarının azaldığını ve reaksiyon sürelerinin ise değişmediğini göstermiştir. MS ve kontrol grubu arasında istatiksel olarak farklılığın bulunmamasının kontrol grubun MS li grubun partnerlerinden oluşması ve bireysel farklılıklardan kaynaklanabileceği düşünülmektedir.
