Persistent Postural Perceptual Dizziness Tedavisinde Vestibüler Rehabilitasyonun Etkisi

Öz

Giriş: Persistent postural-perceptual dizziness (PPPD), son dönemlerde tanımlanan, sürekli bir dizziness, subjektif vertigo ve dengesizlik algısı ile karakterize sinir sisteminin kronik fonksiyonel bir bozukluğudur. Hastaların semptomları özellikle dik duruş pozisyonunda ve karmaşık veya hareketli görsel uyaranların olduğu durumlarda şiddetlenmektedir. Amaç:Bu çalışmanın amacı vestibüler rehabilitasyonun persistent postural-perceptual dizziness tedavisindeki etkisini araştırmaktır. Yöntem: Çalışma kapsamında 18-69 yaş arası (44,3019,50 yaş) 8 kadın 2 erkek toplam 10 bireyin vestibüler değerlendirme ve rehabilitasyon sonuçları incelenmiştir. Hastalara değerlendirme amaçlı okülomotor testler, pozisyonel testler, post head shake nistagmus değerlendirmesi, Fukuda testi, tandem romberg, duyu organizasyon testi (DOT) ve yürüyüş değerlendirmesi yapılmıştır. Vestibüler rehabilitasyon (VR) amaçlı olarak hastalara 3 ay boyunca vestibüler habituasyon egzersizleri verilmiş ve 1 ay aralıklarla kontrol seansına alınmıştır. Bulgular: Hastaların DOT bileşik skor ortalamaları 100 üzerinden VR öncesi 63,5613,89 puan, VR sonrası 74,604,42 puan elde edilmiştir. İki ölçüm arasında istatistiksel olarak anlamlı bir fark elde edilmiştir (p=0,026). Katılımcıların %20’si benign paroksismal pozisyonel vertigo, %10’u vestibüler nörinit sonrası PPPD tanısı almıştır. Hastaların %60’ında stres ve anksiyete de artışla birlikte semptomlarının şiddetlendiği tespit edilmiştir. Sonuç: PPPD hastalarının tedavisinde vestibüler rehabilitasyonun önemi büyüktür. Uygun ve doğru vestibüler rehabilitasyon egzersizleri ile hastaların şikayetlerinde azalma ve denge performansında iyileşme görülmektedir. Bununla birlikte PPPD, vestibüler, nörolojik veya psikiyatrik kaynaklı problemler ile tetiklenebildiği için tedavide bu etkenler de göz önünde bulundurulmalı ve farmakolojik ve psikiyatrik müdahale de PPPD yönetim bataryasına eklenmelidir.

The Effect of Vestibular Rehabılıtatıon in the Treatment of Persistent Postural Perceptual Dizziness

Abstract

Background: Persistent postural-perceptual dizziness (PPPD) is a chronic functional disorder of the nervous system, which is characterized by constant dizziness, subjective vertigo, and a perception of imbalance. The symptoms of the patients are exacerbated especially in the upright position and in the presence of complex or moving visual stimuli. Aim: The aim of this study is to investigate the effect of vestibular rehabilitation in the treatment of persistent postural-perceptual dizziness. Method: Within the scope of the study, the vestibular evaluation and rehabilitation results of 10 individuals, 8 females and 2 males, between the ages of 18-69 (44.3019.50 years) were examined. Oculomotor tests, positional tests, post head shake nystagmus, Fukuda test, tandem romberg, sensory organization test (SOT) and gait evaluation had been performed to the patients. For the vestibular rehabilitation (VR), vestibular habituation exercises were given to the patients for 3 months and they were taken to control sessions at 1-month intervals. Results: The mean DOT composite score of the patients was 63.5613.89 points before VR and 74.604.42 points after VR out of 100. A statistically significant difference was obtained between the two measurements (p=0.026). 20% of the participants were diagnosed with benign paroxysmal positional vertigo and 10% were diagnosed with PPPD after vestibular neurinitis. In 60% of the patients, it was determined that the symptoms were exacerbated with the increase in stress and anxiety. Conclusion: Vestibular rehabilitation is considered of great importance in the treatment of PPPD patients. With appropriate and correct vestibular rehabilitation exercises, patients' complaints decrease and also balance performance improves. However, since PPPD can be triggered by vestibular, neurological or psychiatric problems, these factors should be considered in the treatment and pharmacological and psychiatric intervention should be added to the PPPD management battery.
