TİP 2 DİYABETLİ HASTALARDA GLİSEMİK KONTROL DURUMU VE ETKİLEYEN ETMENLER: DEPRESYONUN ROLÜ

Öz

Araştırmada ayaktan tedavi edilen tip 2 diyabetli hastalarda glisemik kontrol durumunun saptanması, depresyonun ve bazı sosyodemografik, klinik özelliklerin glisemik kontrol üzerine etkisinin belirlenmesi amaçlanmıştır. Kesitsel tipteki araştırma Ocak-Mart 2015 tarihleri arasında bir devlet hastanesi dahiliye polikliniğinde ayaktan tedavi edilen 279 hasta ile yürütülmüştür. Araştırmanın verileri Kişisel Bilgi Formu ve Beck Depresyon Ölçeği ile toplanmıştır. Veri analizinde tanımlayıcı istatistikler, ki kare, Fisher’in kesin testi ve lojistik regresyon analizi kullanılmıştır. Araştırma grubunun %27,2’sinde glisemik kontrolün sağlandığı belirlenmiştir. Lojistik regresyon modeline göre glisemik kontrolün 65 ve üzeri yaş grubunda 2,14 kat, normal Beden Kütle İndeksi’ne sahip olanlarda 1,45 kat, depresyon saptanmayanlarda 1,67 kat yüksek olduğu belirlenmiştir. Araştırmada yaklaşık her dört hastadan birinde glisemik kontrolün sağlandığı, BKİ ve depresyon düzeyinin glisemik kontrolü etkileyen değiştirilebilir birer risk faktörü olduğu belirlenmiştir.Bu doğrultuda koruyucu ruh sağlığı hizmetlerini içerecek şekilde diyabetlilerin nitelikli takibi sağlanmalıdır.

GLYCEMIC CONTROL AND AFFECTING FACTORS IN PATIENTS WITH TYPE 2 DIABETES: THE ROLE OF DEPRESSION

Öz

In this study, it was aimed to determine glycemic control and the effects of depression and some sociodemographic and clinical factors on glycemic control in outpatients with Type 2 Diabetes. This cross-sectional study was conducted with 279 outpatients followed in the internal diseases department of a public hospital between January 2015 and March 2015. The data were collected using the Personal Information Form and Beck Depression Scale. Descriptive statistics, chi-square test, Fisher’s exact test, and logistics regression analysis were used to analyze the data. Glycemic control was achieved in 27.2% of the respondents. According to the logistic regression model, glycemic control was determined to be 2.14 times higher in the patients in the 65 and over age group, 1.45 times higher in those whose Body Mass Index was normal and 1.67 times higher in those having no depression than their counterparts. The results of the study revealed that glycemic control was achieved in one out of four patients, BMI and depression levels were determined to be modifiable risk factors affecting glycemic control. Therefore, quality monitoring of diabetic patients should include preventive mental health care.
