Gebelik ve Emzirme Döneminde COVID-19 Enfeksiyonuna Karşı Aşılanma

Öz

COVID-19 pandemisi ile mücadelede aşı önemli bir yer tutmaktadır. Ancak, aşının fetüs ve bebek üzerindeki bilinmeyen etkileri nedeniyle gebe ve emziren kadınlar aşılarla ilgili klinik deneylerin dışında tutulmuştur. Bu yüzden COVID-19 aşılarının gebelik ve emzirme döneminde kadınlar üzerindeki etkisi hakkında kanıtlar yetersiz olup, bu durum gebe ve emziren kadınlarda endişe ve kaygıya neden olmaktadır. Gebeler, yoğun bakım ünitesine yatış, mekanik ventilasyon ihtiyacı ve ölüm dâhil olmak üzere COVID-19 enfeksiyonunun ciddi sonuçları açısından normal popülasyona göre riskli grup olarak kabul edilse de aşılanma sürecinde öncelikli gruplar arasında yer almamakta, aşının yararının potansiyel riskten daha ağır bastığı durumlarda gebelerin kendi istekleri dahilinde aşılanması önerilmektedir. Benzer şekilde emziren kadınlarda COVID-19 aşılarının güvenliği, aşılanmanın anne sütüyle beslenen bebeğe ve laktasyona etkileri konusunda yeterli veri bulunmasa da emziren kadınlar veya bebekleri için bir risk oluşturmadığı bildirilmekte ve emziren kadınlara COVID-19 aşısı önerilmektedir. Aşılarla ilgili gebe ve emziren kadınların karar vermesinin desteklenmesi için aşılar hakkında kanıta dayalı ve kişiye özel bilgilere gerek vardır ve bu bilgiler ebe, hemşire ve hekim gibi sağlık profesyonelleri tarafından sağlanmalıdır. Bu bağlamda bu derleme, gebelik ve emzirme döneminde COVID-19 enfeksiyonuna karşı aşılanmayı bilimsel veriler ve literatür ışığında ele almak amacıyla yapılmıştır.

Vaccination Against Coronavirus Infection During Pregnancy and Breastfeeding Period

Öz

Vaccination plays an important role in combating the COVID-19 pandemic. However, pregnant women and lactating women are excluded from clinical trials of vaccines due to the unknown effects of the vaccine on the fetus and infant. Therefore, there is insufficient evidence about the effect of COVID-19 vaccines on women during pregnancy and the breastfeeding period which causes concern and anxiety among pregnant women and lactating women. Although pregnant women are considered to be a risk group compared to the normal population in terms the serious consequences of the infection, including hospitalization in the intensive care unit, need for mechanical ventilation and death, they are not among the priority groups in the vaccination process. In cases where the benefit of the vaccine outweighs the potential risk, it is recommended that pregnant women be vaccinated on their own will. Similarly, although there is insufficient data on the safety of COVID-19 vaccines among breastfeeding women and the effects of vaccination on breastfed babies and lactation, it is reported that COVID-19 vaccines do not pose a risk for breastfeeding women or their babies, and thus COVID-19 s vaccine is recommended for breastfeeding women. Evidence-based and personalized information about vaccines is needed to support pregnant and breastfeeding women's decision making about vaccines, and this information should be provided by health professionals such as midwife, nurse and physicians. In this context, this review was conducted to discuss vaccination against COVID-19 infection during pregnancy and the breastfeeding period based on scientific data and literature.
