Pes Ekinovaruslu Olguların Ponseti Yöntemi İle Tedavi Sonuçları

Öz

Amaç: Ponseti yöntemi ile tedavi edilen Pes Ekinovarus (PEV) deformitelerinin sonuçlarını literatürdeki çalışmalar eşliğinde irdelemek. Gereç ve Yöntem: Retrospektif yapılan bu çalışmaya PEV deformiteli 35 hastanın toplam 54 ayağı dahil edildi. Hastalar alçılama öncesi Dimeglio skorlama sistemine göre değerlendirildi. Tüm hastalara tek bir hekim tarafından Ponseti seri alçılama metodu ve 41 ayağa perkütan aşilotomi uygulandı. Alçı sonrası Dennis Brown ortezi kullanıldı. Ayakların son durumu ICFSG değerlendirme formuna göre değerlendirildi. Bulgular: Çalışmada 13 kız 22 erkek toplam 35 hastanın 54 ayağı değerlendirildi (16 unilateral, 19 bilateral). Ponseti öncesi Dimeglio skorları; 2 ayak grade-1, 15 ayak grade-2, 24 ayak grade-3, 13 ayak grade-4’dü. Hastaların 6 tanesinde nöral tüp defekti veya Down Sendromu bulunmaktaydı. Ortalama 6 (4-10) kez alçılama yapıldı ve 30 (12-60) ay takip edildi. Başlangıç yaşı 7 gün ile 8 ay arasında değişmekteydi. Sonuç: International Club Foot Study Group değerlendirme formuna göre 38 (%70,4) ayakta mükemmel, 10 (%18,5) ayakta iyi ,4 (%5,7) ayakta yetersiz, 2 (%3,7) ayakta ise kötü sonuç elde edilmiştir.
