Erişkin ve Adölesan Asetabüler Displazide Ganz Osteotomisi Sonuçları

Öz

Amaç: Erişkin ve adölesan asetabüler displazi neonatal kalça instabilitesinin uzun dönem sonucu olarak meydana gelen önemli bir sorundur. İzole asetabüler displazi terimi asetabulumun konkavitesinin azaldığı, oblisitesinin arttığı fakat kalça uyumunun dislokasyona yol açmayacak düzeyde korunduğu durumu tanımlar.
Semptomatik asetbüler displazi olgularında ağrının giderilmesi ve sekonder osteartrit gelişiminin önlenmesi için tedavi gereksinimi ortaya çıkabilmektedir. Çalışmamızla semptomatik asetabüler displazi tanısıyla Ganz osteotomisi operasyonu gerçekleştirilmiş olguların erken dönem sonuçlarını ortaya koymayı amaçladık.
Gereç ve Yöntem: Haziran 2005 ve Temmuz 2014 tarihleri arasında asetabüler displazi tanısı ile Ganz osteotomisi operasyonu gerçekleştirilmiş olan hastalar geriye dönük belirlenmiştir. Preoperatif ve postoperatif dönemde Harris Kalça Skoru, asetabüler indeks, merkez kenar açısı (MKA) ve anterior merkez kenar açısı (AMKA) hesaplanarak kayıt altına alındı.
Bulgular: Ameliyat öncesi ve sonrası değerlerdeki değişim değerlendirildiğinde, Harris Kalça Skoru, asetabüler indeks, CEA ve ACEA değerlerinde anlamlı iyileşme olduğu tespit edildi. (p<0,001)
Sonuç: Klinik ve radyolojik skorlar değerlendirildiğinde, Ganz osteotomisinin erken-orta dönem sonuçlarının memnuniyet verici olduğu görüldü.

Ganz Osteotomy Results in Adult and Adolescent Acetabular Dysplasia

Öz

Objectives: Adult and adolescent acetabular dysplasia is an important problem that occurs as a long-term consequence of neonatal hip instability. The term isolated acetabular dysplasia defines the condition in which the concavity of the acetabulum decreases, its obliquity increases, but the hip compliance is maintained at a level that does not cause significant dislocation.
In symptomatic cases of acetbular dysplasia, treatment may be required to relieve pain and prevent the development of secondary osteoarthritis. In our study, we aimed to present the early results of patients who underwent Ganz osteotomy with the diagnosis of symptomatic acetabular dysplasia. Material and Methods: Patients who underwent Ganz osteotomy with a diagnosis of acetabular dysplasia between June 2005 and July 2014 were retrospectively identified.
Harris Hip Score, acetabular index, center edge angle (CEA), anterior center edge angle (ACEA) were measured both preoperative and postoperative period. Results: When the preoperative and postoperative values were compared, it was seen that there was a significant change in Harris Hip Score, acetabular index, CEA and ACEA values (p<0,001). Conclusion: When the clinical and radiological scores were evaluated together, it was seen that the early-mid-term results of Ganz osteotomy were satisfactory.
