Chronic Kidney Disease Associated Pruritus: Evidence for Systematic Assessment in Nephrology Care - A Scoping Review/Kronik Böbrek Hastalığı ile İlişkili Kaşıntı: Nefroloji Bakımında Sistematik Değerlendirme Kanıtları - Bir Kapsam Belirleme İncelemesi

Chronic kidney disease-associated pruritus is a debilitating symptom with high prevalence in end-stage renal disease. Despite its severity, it is frequently undervalued and underdiagnosed due to the absence of systematic assessment protocols and patient underreporting. This review maps the evidence regarding the necessity of transitioning from subjective observation to standardized, systematic evaluation in nephrology care. A scoping review was conducted following the Joanna Briggs Institute methodology and the Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews checklist. Databases MEDLINE (PubMed) and CINAHL were searched for records published between 2016 and 2026, including projections, focusing on adults undergoing hemodialysis. Fifteen studies were included. Findings demonstrate that instruments such as the Worst Itch Numeric Rating Scale and the 5-D Itch Scale possess high clinical sensitivity and reliability, significantly increasing the detection of severe cases that clinical teams, often focused on biochemical markers, typically overlook. A persistent discrepancy exists between healthcare professionals perceptions and the actual symptomatic burden experienced by people with chronic kidney disease. Evidence strongly supports the integration of systematic assessment protocols into routine nephrology nursing. Utilizing validated instruments is the essential starting point for early symptom recognition, facilitating timely interventions and improving patient quality of life.
