Short-term effects of tirzepatide on weight loss in treated primary hypothyroidism: a retrospective study

Öz

Objective: This study examined whether well-controlled primary hypothyroidism treated with levothyroxine affects short-term body mass index reduction and metabolic responses to tirzepatide in adults with overweight or obesity. Material and Method: In this retrospective, single-center study, adults with overweight or obesity initiating tirzepatide were divided into two groups: individuals without thyroid disease and those with primary hypothyroidism receiving stable levothyroxine. All participants received once-weekly tirzepatide for one month along with standard lifestyle counseling. Measurements at baseline and one month included body mass index, weight, fasting blood glucose, glycated hemoglobin, thyroid-stimulating hormone, free thyroxine, and lipid parameters. Within-group changes were analyzed using paired tests, while between-group comparisons were evaluated with independent tests and repeated-measures analyses. Results: Both groups showed significant decreases in body mass index and weight, with improvements in fasting blood glucose, glycated hemoglobin, and lipid profile. Thyroid-stimulating hormone and free thyroxine remained stable and did not differ between groups. No thyroid-related adverse events occurred. The degree of body mass index reduction was similar regardless of levothyroxine use. Conclusion: Well-controlled primary hypothyroidism treated with levothyroxine does not reduce short-term body mass index loss or early metabolic benefits of tirzepatide, indicating that tirzepatide is effective regardless of thyroid hormone replacement.
