Postpartum Dönemde Başlayan Fagofobi: Bir Olgu Sunumu

Öz

Fagofobi, yutma fobisi olarak da adlandırılan ve altta yatan herhangi bir organik patoloji saptanmaksızın yutma güçlüğü ile seyreden bir özgül fobi türüdür. Çoğu olguda stresli yaşam olayları ya da travmatik yutma deneyimlerinin ardından gelişir ve öğrenilmiş kaçınma davranışıyla sürdürülür. DSM-5 sınıflandırmasında özgül fobiler başlığı altında yer almaktadır. Yeme davranışının kısıtlanmasına bağlı olarak kişide yetersiz beslenme görülebilir ayrıca buna eşlik eden yoğun anksiyete, belirgin fiziksel ve psikolojik sorunlara yol açabilir. Ayırıcı tanıda, yutma güçlüğünün yalnızca besin alımı sırasında ortaya çıkması konversiyon bozukluklarından ayrımda önem taşırken; beden algısında bozulma ya da zayıflama amacının bulunmaması ise yeme bozukluklarından ayırt edilmesini sağlar. Bu olgu sunumunda, fagofobisi doğum sonrası dönemde başlayan ve altı yıldır devam eden bir hastanın ele alınmasıyla literatüre katkı sağlanması hedeflenmiştir.

Phagophobia Onset in the Postpartum Period: A Case Report

Öz

Phagophobia, also known as swallowing phobia, is a type of specific phobia characterized by difficulty swallowing in the absence of any underlying organic pathology. It commonly develops following stressful life events or traumatic swallowing experiences and is maintained by learned avoidance behaviors. In the DSM-5 classification, it is categorized under specific phobias. Restriction of eating behavior may result in inadequate nutrition, and the accompanying intense anxiety can lead to significant physical and psychological symptoms. In the differential diagnosis, the presence of swallowing difficulty exclusively during food intake is important in distinguishing phagophobia from conversion disorders, while the absence of body image disturbance or weight-loss-related behaviors helps differentiate it from eating disorders. This case report aims to contribute to the literature by presenting a patient whose phagophobia began in the postpartum period and has persisted for six years.
