RCA of Left Sinus of Valsalva Origin: A Rare Case of Coronary Anomaly

Öz

Coronary artery anomalies (CAAs) include congenital conditions characterized by abnormal origin or course of any of the 3 major coronary arteries. The course of the epicardial arteries can vary significantly between individuals. Coronary angiography performed on a 63-year-old female patient who applied to the emergency department with angina and dyspnea revealed that right coronary artery (RCA) originated from the left sinus of Valsalva. Following the procedures, the patient was discharged with medical treatment. No invasive intervention was needed. Most CAAs have no clinical significance, but some may have potential serious sequelae. The fact that the RCA originates from the left and courses between the aorta and the pulmonary artery is called "malignant course". It should not be forgotten that this anomaly can cause angina, dyspnea, and more importantly, sudden cardiac death in the absence of atherosclerosis.

Öz

Koroner arter anomalileri (KAA'lar), 3 ana koroner arterden herhangi birinin anormal kökeni veya seyri ile karakterize edilen konjenital durumları içerir. Epikardiyal arterlerin seyri önemli ölçüde bireyler arası değişkenlik gösterebilir. 63 yaşında anjina, dispne ile acil servise başvuran kadın hastada yapılan koroner anjiografide Sağ koroner arter’in (RCA) sol sinüs valsalvadan köken aldığı görüldü. Hasta yapılan işlemlerin ardından medikal tedavi düzenlenerek taburcu edildi. Girişimsel müdahaleye ihtiyaç duyulmadı. KAA’ların çoğunun klinik önemi yoktur ancak bazılarında potansiyel ciddi sekeller olabilmektedir. RCA’nın sol kökenli olması ve aort ile pulmoner arter arasında seyretmesi "malign seyir" olarak adlandırılır. Bu anomalinin ateroskleroz yokluğunda anjina, dispne ve daha önemlisi ani kardiyak ölüme neden olabileceği unutulmamalıdır.
