ASSESSMENT OF SLEEP DISORDERS AND QUALITY OF LIFE IN CHILDREN WITH PRIMARY HEADACHE

Öz

Introductıon: Headache is a common disorder that affects children as well as adults. Sleep-related disorders are shown to be more often in children with headache. This is an important problem that can deteriorate the quality of life in addition to worsening the natural course of the disease. The aim of this study was to evaluate sleep disorders and quality of life in children with primary headache.

Materials and Methods: This study was carried out among the patients who applied to Pediatric Department and Pediatric Neurology outpatient clinics of Trakya University Faculty of Medicine. The study included 33 primary headaches and 36 healthy volunteers aged 6-18 years. Demographic and clinical datas were gathered for all children. In order to evaluate the sleep disturbances a ‘Pediatric Sleep Questionnaire’ and to evaluate the quality of life ‘Pediatric Quality of Life Inventory’ were applied to all patients and control group.

Results: The patient group consisted of 33 patients (25 had migraine, 5 had tension headache, 3 had migraine and tension headache). Behavioral problems, breathing problems, other scores, total sleep problems score and total score were significantly higher in the patient group than the control group. Sleep-related movement disorders, difficulty in falling asleep, falling asleep and sleep problems, sleep waking frequency, nightmare disorders, additional problems, daytime sleepiness, bruxism, morning waking up tired, restless sleep and sleep talking in the patient group were statistically significantly higher than control group. There were no diffferences between the two groups in terms of snoring score, sleep sweating and sleepwalking. All quality of life scores were significantly lower in the patient group than the control group.

Conclusion: In conclusion this study demonstrates the characteristics of sleep disorders and quality of life in children with primary headache. Children with primary headache have an increase in the frequency of sleep disorders and a decrease in quality of life. Therefore, questioning sleep disorders and quality of life should be part of the evaluation of children with primary headache.

PRİMER BAŞ AĞRISI OLAN ÇOCUKLARDA UYKU BOZUKLUKLARI VE YAŞAM KALİTESİNİN DEĞERLENDİRİLMESİ

Öz

Giriş: Baş ağrısı erişkinler kadar çocukları da etkileyen ve sık karşılaşılan bir durumdur. Baş ağrısı olan çocuklarda uyku ile ilişkili bozuklukların daha sık olduğu yapılan çalışmalarla gösterilmiştir. Baş ağrılarına eşlik eden uyku bozukluklarının sık olması, hastalığın doğal seyrini kötüleştirmesinin yanı sıra yaşam kalitesini de etkilemektedir. Çalışmamızda primer baş ağrısı olan çocukların uyku bozuklukları ve yaşam kalitesinin değerlendirilmesi amaçlanmıştır.

Gereç ve Yöntem: Bu çalışma Trakya Üniversitesi Tıp Fakültesi Çocuk Sağlığı ve Hastalıkları Anabilim Dalı Çocuk Polikliniği ve Çocuk Nöroloji Polikliniklerine başvuran hastalar arasında yapıldı. Çalışmaya 6-18 yaş arası 33 primer baş ağrısı ile aynı yaş aralığında 36 sağlıklı gönüllü olmak üzere toplam 69 çocuk dahil edildi. Çalışmaya alınan çocukların demografik ve klinik verileri sorgulandı. Uyku bozukluklarını değerlendirmek amacıyla ‘Çocuklarda Uyku Ölçeği’, yaşam kalitesini değerlendirmek amacıyla da ‘Çocuklar İçin Yaşam Kalitesi Ölçeği’ uygulandı.

Bulgular: Hasta grubuna alınan 33 hastanın 25’ini migren, 5’ini GTBA, 3’ünü migren ve GTBA oluşturdu. Hasta grubunda davranış sorunları, nefes alma sorunları, diğer puan, toplam uyku sorunları puanı ve toplam puan kontrol gruba göre anlamlı olarak yüksek bulundu. Uyku ilişkili hareket bozuklukları, uykuya dalmada güçlük, uykuya dalma ve uykuyu devam ettirme sorunları, uykudan uyanma sıklığı, kabus bozuklukları, nefes alma sorunlarına ilave sorunlar, gün içi uykululuk, diş gıcırdatma, sabah yorgun uyanma, huzursuz uyku ve uykuda konuşma hasta grubunda kontrol gruba göre istatistiksel olarak anlamlı yüksek saptandı. Horlama puanı, uykuda terleme ve uyurgezerlik açısından iki grup arasında farklılık bulunmadı. Tüm yaşam kalitesi skorları hasta grubunda kontrol grubuna göre anlamlı olarak düşük bulundu.

Sonuç: Sonuç olarak bu çalışma ile primer baş ağrısı tanılı çocuklarda uyku bozuklukları ve yaşam kalitesi arasındaki ilişki gösterilmiştir. Primer baş ağrısı olan çocuklarda uyku bozukluğu sıklığında artış, yaşam kalitesinde düşüş olmuştur. Bu sebeple uyku bozuklukları ve yaşam kalitesinin sorgulanması, primer baş ağrılı çocukların değerlendirilmesinin bir parçası olmalıdır.
