Tiroid ince iğne aspirasyon biyopsilerinin histopatolojik sonuçlar ile retrospektif olarak değerlendirilmesi Türkçe

Öz

Amaç: Tiroid ince iğne aspirasyon biyopsi sonuçları ile opere edilen olguların histopatoloji sonuçları karşılaştırılarak, tiroid lezyonlarının değerlendirilmesinde ince iğne aspirasyon biyopsilerinin etkinliğinin saptanması amaçlanmıştır.Materyal ve metod: Harran Üniversitesi Tıp Fakültesi polikliniklerinde 2008-2012 yılları arasında nodüler tiroid lezyonu nedeniyle tiroid ince iğne aspirasyon biyopsi uygulanan ve devamında tiroid rezeksiyonu yapılan 84 olgu çalışmaya dahil edildi. Her olguya ait patoloji raporu arşivden çıkarıldı. Sitolojik değerlendirmede tiroid için kullanılan Bethesta sisteminin esas alındığı görüldü. Buna göre olgularımız yetersiz, benign, malignite şüpheli ve malign kategoride toplandığı saptandı. Bethesta sisteminde bulunan foliküler neoplazi/şüphesi ile önemi belirlenemeyen atipi/foliküler lezyon kategorisinde olgumuzun olmadığı belirlendi. Sitolojik tanılar rezeksiyon materyallerinin histopatolojik sonuçlarıyla karşılaştırıldı.Bulgular: Hastaların 22'si % 26,2 erkek olup, yaş ortalamaları 50,0±3,7 idi. Kadın hasta sayısı ise 62 %73,8 olup, yaş ortalamaları 44,6±1,5 idi. Sitolojik tanı dağılımı 67'si % 79,8 benign, 9'u % 10,7 malignite şüpheli, 3'ü % 3,6 malign, 5'i %6 ise yetersiz materyal şeklinde idi. Rezeksiyon sonuçları ile karşılaştırıldığında, sitolojisi benign olarak değerlendirilen olguların 13'ünde %19,4 , malignite şüpheli olarak değerlendirilen olguların 7'sinde %77,8 , malign olarak değerlendirilen olguların tamamında %100 , yetersiz olarak değerlendirilen olguların 1'inde % 20 histopatolojik incelemede malignite saptandığı görüldü. Bu sonuçlar ışığında tiroid ince iğne aspirasyon biyopsisi'nin benign tanısındaki başarısı % 80,6, malign tanısında başarısı % 100, testin tanı değeri % 76,2, yanlış negatiflik oranı % 19,4, yanlış pozitiflik oranı ise % 0,0 olarak saptandı. Şüpheli olgular pozitif kabul edildiğinde özgüllük %96,5 duyarlılık % 43,5 olarak saptandı.Sonuç: Tiroid ince iğne aspirasyon biyopsisi etkin ve güvenilir bir tanı yöntemidir ve tiroid nodüllerine yaklaşımda büyük öneme sahiptir

Öz

Background: Aimed to determine the utility of fine needle aspiration biopsy in the evaluation of thyroid lesions at our faculty comparing the results of fine needle aspiration biopsy and pathology report results of the patients who underwent operation.Methods: At the Harran University School of Medicine clinics between 2008 and 2012, 84 cases that had performed fine-needle aspiration biopsy of thyroid and underwent thyroidectomy due to nodular thyroid lesions were included in the study. The pathology report of each patient was removed from the archive. It was seen that Bethesta system, which is based on cytologic evaluation was used for thyroid. Accordingly, it was detected that the cases were collected in inadequate, benign, and suspicious for malignancy and malignant category. In Bethesta atypia/follicular lesion of undetermined significance and suspicious for follicular neoplasm/follicular neoplasm category, our case was not identified. The cytologic results were compared with histopathologic results.Results: Twenty-two patients 26.2% were male and mean age was 50.0±3.7. The number of female patients 62 73.8% and the mean age was 44.6 ± 1.5. Cytological results were reported as 67 % 79.8 benign, 9 % 10.7 malignant suspicious, 3 % 3.6 malignant and 5 %6 inadequate materials. Compared with the results of resection, 13 patients diagnosed as benign cytology 19.4% , 7 patients diagnosed as suspicious for malignancy 77.8% , all patients diagnosed as malignant 100% , 1 patient was considered to be insufficient % 20 were found to be malignant on histopathologic examination. Based on these results the success of fine needle aspiration biopsy of thyroid 80.6% in the diagnosis of benign and malignant diagnosis 100% success rate and diagnostic value of 76.2%, 19.4% false negative rate, false positive rate was 0.0%. Considered positive when cases of suspected sensitivity of 96.5% and specificity 43.5%.Conclusions: Fine needle aspiration biopsy of thyroid is an effective and reliable diagnosing method and management of thyroid nodules is of great importance
