Videolaringoskopi ve Luc Forseps Kullanarak Larinkste Horizontal Yerleşimli Tavuk Kemiğinin Çıkarılması

Öz

Üst solunum yollarında yer alan yabancı cisimler, yaşamı tehdit edebilecek ciddi sonuçlara yol açma ihtimali olan acil durumlardır. Bu tür cisimlere doğru yöntem ile yaklaşılmadığında, olası aspirasyon ve üst solunum yolunda tam obstruksiyon gelişmesi durumunda hayatı tehdit eden durumlar oluşmaktadır. Bu tür cisimler yetişkin larenksinde nadiren görüldüğünden, durumu tanımlamanın ve bunların çıkarılması için rasyonel bir yöntem sunmanın yararlı olduğu ortadadır. Tavuk kemiği yutmuş 64 yaşında bir kadın hastanın vakası sunularak bu durumlarda kullanılabilecek bir yöntem ortaya koymak bu vaka sunumunun temel amacıdır. Cisim tam glottik seviyede horizontal olarak yerleştiğinden ve entübasyonu çok zorlaştırdığından, videolaringoskop yardımıyla çıkarıldı. Bu vaka sunumu, preoperatif doğru değerlendirmeyi, intraoperatif hava yolu yönetimini ve yabancı cismin çıkarılmasını göstermektedir. Videolaringoskopi rehberliğinde forsepsle cismin çıkarılması, özellikle glottik seviyeye yakın laringeal yabancı cisimlerin çıkarılmasını içeren uygun vakalarda güvenli bir yöntemdir. Bu yöntem, zayıf hasta kooperasyonu veya uyumu, yüksek aspirasyon riski veya entübasyonun tehlikeli olduğu düşünülen vakalarda (özellikle laringeal yabancı cisimlerin varlığında) güvenle kullanılabilir.

Using Videolaryngoscopy and Luc Forceps to Remove a Transversely Located Chicken Wishbone in the Larynx

Öz

The presence of a foreign body in the upper airways is a serious condition that can be life-threatening. If such foreign bodies are not managed using the appropriate technique, there is a risk of life-threatening complications due to potential aspiration and complete upper airway obstruction. Because such bodies are seldom seen in the adult larynx, we consider it useful to describe the condition and offer clinicians a rational method for their removal. We report the case of a 64-year-old female patient who had swallowed a chicken wishbone. Because the object was located transversely at the whole glottic level, making intubation very difficult, it was removed with the help of a videolaryngoscope. The report illustrates successful laryngoscopy preoperative evaluation, operative airway management, and chicken wishbone removal. Removal of the object with forceps, guided by videolaryngoscopy, is a safe method in appropriate cases, especially those involving the removal of laryngeal foreign bodies close to the glottic level. This method can be safely employed in cases involving poor patient cooperation or compliance, high aspiration risk, or when intubation is considered hazardous, particularly in the presence of laryngeal foreign bodies.
