Subksifoid Uniportal Cerrah Destekli Robotik Akciğer Lobektomisi: Olgu Sunumu

Öz
Amaç: Akciğer kanserlerinde erken evrede ana tedavi cerrahi rezeksiyondur. Ancak video torasik yardımlı torasik cerrahi (VATS) tekniğinin ameliyat esnasında bazı kısıtlılıkları bulunmaktadır. Bu nedenle robotik cerrahi teknik ile VATS’ın birleşimi olan cerrah destekli robotic cerrahi (SpRS) enstrümanlar kullanılmaya başlanmıştır. Bu makalede SpRS ile yaptığımız bir subskifoid sağ üst lobektomi olgusu sunulmuştur. Olgu: Atmış üç yaşında erkek hasta, iki yıl önce COVID-19 şüphesi ile çekilen toraks bilgisayarlı tomografisinde saptanan bir lezyonun takipleri sırasında, sağ akciğer üst lobda yeni gelişen soliter pulmoner nodül nedeniyle tarafımıza başvurdu. Hastaya çekilen pozitron eminisyon tomografisinde (PET-BT) mevcut lezyonda yüksek FDG tutulumunun da olması üzerine hastaya cerrah destekli robotik subksifoid sağ üst lobektomi yapıldı. Sonuçlar: Minimal invaziv cerrahi, gelişen teknolojiler ile birlikte, toraktomi ile yaptığımız ameliyatları çok daha küçük kesiler ile gerçekleştirmeyi sağlamaktadır. Cerrah destekli robotik cerrahi aletler ile subksifoid rezeksiyonlar daha kolay ve onkolojik prensiplere uygun olarak uygulanabilmektedir.

Background:
Surgical resection is the first choice of treatment in early-stage lung cancer. Video thoracoscopy Assisted Thoracic Surgery (VATS) has limitations in surgery; therefore, Surgeon Powered Robotic Surgery (SPRS), a hybrid of robotic surgery and VATS, is recently preferred. This study describes a subxiphoid right upper lobectomy with SPRS technology.
Case:
A 63-year-old male patient under routine observation for 2 years because of a nodule was referred to our clinic because of a recently developed solitary pulmonary nodule located in the right upper lobe seen in a recent thoracic computed tomography (CT). The patient underwent subxiphoid right upper lobectomy with SPRS technology. A staging positron emission tomography-computed tomography (PET-CT) was performed, and high FDG uptake was reported.
Result:
Due to technological advancements in minimally invasive surgery, the operations can be completed from smaller incisions than a thoracotomy. Subxiphoid resections can be more accessible while following the oncological principles when SPRS instruments are used.
