Tele-Tıp Uygulamalarının Donabedian Modeline Göre Kalite Perspektifinden İncelenmesi

Abstract

Bu çalışmanın amacı, Tele-tıp uygulamalarının Donabedian’ın yapı, süreç ve sonuç modeline göre kalite perspektifinden uygulanabilirliğinin incelenmesi ve Tele-tıp hizmetlerinin hangi yapısal gerekliliklere ihtiyaç duyduğunun belirlenmesidir. Donabedian’ın yapı, süreç ve sonuç modeli açıklandıktan sonra Tele-tıp kavramından bahsedilmiştir. Son aşamada ise Tele-tıp uygulamaları Donabedian modeline göre kalite perspektifinden incelenmiştir. Çalışmanın sonucunda Donabedian’ın yapı, süreç ve sonuç modeline göre kalite boyutları farklılaşırken temel olarak tüm süreçlerde önemli olan boyutların erişilebilirlik, hakkaniyet, uygunluk, etkinlik ve verimlilik olduğu görülmüştür. Tele-tıp faaliyetleri Donabedian Model’ine göre incelendiğinde yapı boyutunda bina, donanım ve fiziksel koşulların Tele-tıp faaliyetlerinin etkinliği için düzenlenmelerin yapılması gerektiği ve sağlık hizmeti sunucularının, Tele-tıp faaliyetleri ile ilgili eğitim alan ve sürece hâkim kişilerden oluşması gerektiği görülmüştür. Süreç boyutunda, Tele-tıp uygulamalarının kullanılması ile gereksiz işlem maliyetlerinin azaltıldığı ve erken teşhisler ile hastalığın ilerlemesinin önüne geçildiği görülmüştür. Sonuç boyutunda ise yapılan hatalar Tele-tıp uygulamaları ile minimize edilirken hastaların hastanede kalış sürelerinin azaldığı görülmüştür. Sonuç olarak Tele-tıp faaliyetlerini kalite perspektifinden geliştirmede sağlık profesyonelleri için eğitim ve sertifikasyon programlarının geliştirilmesi, güncel ve güvenilir teknolojik altyapı kullanılması, diğer sağlık sistemleri ile entegre edilmesi, hukuki ve etik standartların çerçevesinin belirlenmesi, faaliyetlerin sürekli olarak izlenmesi, hatalardan öğrenme ve sürekli gelişim için geri bildirim döngülerinin kullanılmasının gerektiği anlaşılmıştır.

Examination of Telemedicine Applications from the Perspective of Quality According to the Donabedian Model

Abstract

The aim of this study is to examine the applicability of Tele-medicine applications from a quality perspective according to Donabedian's structure, process and outcome model and to determine which structural requirements Tele-medicine services need. After Donabedian's structure, process and outcome model is explained, the concept of Tele-medicine is mentioned. In the last stage, Tele-medicine applications were analysed from a quality perspective according to the Donabedian model. As a result of the study, while the quality dimensions differ according to Donabedian's structure, process and outcome model, it is seen that the important dimensions in all processes are accessibility, equity, appropriateness, effectiveness and efficiency. When tele-medicine activities are analysed according to the Donabedian Model, it is seen that in the structure dimension, building, equipment and physical conditions should be arranged for the effectiveness of tele-medicine activities and health service providers should consist of people who are trained in tele-medicine activities and have a good command of the process. In the process dimension, it was observed that the use of telemedicine applications reduced unnecessary transaction costs and prevented the progression of the disease with early diagnoses. In the outcome dimension, it was observed that the errors made were minimised with Tele-medicine applications and the length of hospital stay of the patients decreased. As a result, it is understood that in developing telemedicine activities from a quality perspective, it is necessary to develop training and certification programmes for health professionals, use up-to-date and reliable technological infrastructure, integrate with other health systems, determine the framework of legal and ethical standards, continuously monitor activities, learn from mistakes and use feedback loops for continuous improvement.
