Streptokoksik Tonsillofarenjit Tanısında Strep - A Hızlı Testinin Etkinliği

Öz

Amaç: Streptokok tonsillofarenjitinin erken tanısında Strep-A hızlı aglütinasyon testinin etkinliğinin belirlenmesi amaçlandı. Yöntem: Hastanemize Haziran 2007- Temmuz 2008 döneminde başvuran ve klinik bulguları ile streptokok tonsilllofarenjiti düşündüren 132 vakada boğaz sekresyonunda Strep-A hızlı tanı testi ilk adımda yapıldı. Strep-A aglütinasyon testinde VIKIA Strepto A kiti kullanıldı. Doğrulama testi olan boğaz kültürü de standart yöntemle yapıldı. Bulgular: Araştırmadaki 132 vakadan 33 % 25 ’ünde boğaz kültüründen A grubu β-hemolitik streptokok izole edildi. Streptokoksik tonsillofarenjit tanısı alan 33 vakadan 22 % 66 ’sinde Strep-A hızlı tanı testi pozitif, 11 % 34 ’inde negatif saptandı. Boğaz kültüründe üremesi olmayan bir vakada Strep A hızlı tanı testi pozitif saptandı. Strep-A hızlı tanı testi streptokok tonsillofarenjiti tanısında % 66 duyarlılığa, % 99 özgüllüğe sahiptir. Sonuç: Strep-A hızlı tanı testi pozitif saptandığında streptokoksik tonsillofarenjit hastalığını gösterirken, testin negatif saptanması tanıdan uzaklaştırmaz.

The Effectiveness of Rapid Strep A Test in the Diagnosis of Streptococcal Tonsillopharyngitis

Öz

Aim: We aimed to determine the efficacy of Strep-A rapid agglutination test in the early diagnosis of streptococcal tonsillopharyngitis. Methods: Rapid antigen detection tests were performed as a first step in 132 cases who referred to our hospital between June 2007, and July 2008 with clinical findings suggestive of streptococci tonsillopharyngitis. VIKIA Strepto A kit was used for Strep-A agglutination test. Confirmatory throat culture was performed using standard procedures. Results: Group A β-hemolytic streptococci were isolated from the throat cultures of 33 patients 25 % out of the 132 cases with streptococcal tonsillopharyngitis. While, 22 66 % patients had Strep-A positivity, and 11 33 % of them had Strep-A negativity in the rapid antigen detection test. Strep A rapid antigen detection test was positive in one case whose throat culture showed no streptococci. Strep-A rapid detection test in the diagnosis of streptococcal tonsillopharyngitis has a sensitivity of 66 %, and a specificity of 99 %. Conclusions: Although positive Strep-A rapid detection test shows the presence of a streptococcal tonsillopharyngitis, its negativity does not distract us from the possibility of the diagnosis.
