Göbek Kordonu Alfa-Fetoprotein Düzeyi ile Neonatal Hiperbilirubinemi İlişkisi

Öz

Amaç: Neonatal hiperbilirubinemi yenidoğanların en sık karşılaşılan sorunlarından biridir. Ciddi indirekt hiperbilirubinemi yetersiz tedavi yapıldığında nörotoksisiteye neden olabilir. Ancak, hiperbilirubineminin gelişmesini ve ağırlığını öngören bir gösterge henüz bulunamamıştır. Alfa-fetoproteinin AFP neonatal hiperbilirubineminin ciddiyetini öngörebileceğini destekleyen çalışmalar mevcuttur. Bu az sayıdaki ve az sayıda bebekle yapılan çalışmaların sonuçları çelişkilidir. Bu çalışmada kordon kanı AFP düzeyi ile neonatal hiperbilirubinemi ilişkisi araştırıldı. Yöntem: Çalışmaya, Mart 2005-Ekim 2005 ayları arasında doğan 504 miadında yenidoğan alındı. Coombs pozitif Rh uygunsuzluğu ve ABO uygunsuzluğu veya hemolitik sarılığı olan; asfiksi, konjenital anomali, damar dışına kanama bulgusu saptanan yenidoğanlar çalışmaya alınmadı. Vakaların kordon kanında AFP düzeyi ölçüldü. Sarılık gelişenlerde hemen, diğerlerinde ise taburculukları sırasında tarama testi alınması sırasında kapiller bilirubin düzeyine bakıldı. Bulgular: Ortalama göbek kordonu AFP düzeyi 49.1±44.9 mg/L 1.1-396.2 mg/L , ortalama total serum bilirubin TSB değeri 5.8 ±3.1 1-19.4 mg/dL, ortalama bilirubin bakılma yaşı 37±23.2 12-144 saat olarak saptandı. Vakaların göbek kordonu AFP ve TSB düzeyleri arasında anlamlı pozitif korelasyon bulundu. Ancak, bu korelasyon zayıf derecedeydi r=0.187 p

Relationship Between Neonatal Hyperbilirubinemia and Umbilical Cord Alpha Fetoprotein Level

Öz

Objective: Neonatal hyperbilirubinemia is one of the most common problems in the newborn period. Severe indirect hyperbilirubinemia can cause neurotoxicity if not treated properly. But there isn’t any test to predict the development severe hyperbilirubinemia. There are few studies with few patients suggesting that alpha-fetoprotein AFP levels can predict severity of hyperbilirubinemia. But the results of these studies are inconsistent. In our study, the relationship of umbilical cord AFP level and neonatal hyperbilirubinemia was investigated. Methods: Five hundreds and four full term newborns were included in the study. Newborns with Coombs positive Rh or ABO incompatability, hemolytic jaundice, asfixia and congenital anomalies were excluded from the study. AFP levels were measured from blood samples derived from umbilical cords of the patients. Capillary blood samples for bilirubin level determinations were drawn from each newborn along with the routine phenylketonuria screening, but immediately from newborns with jaundice. Results: Mean umbilical cord AFP level, mean total serum bilirubin level TSB and mean age at which the bilirubin level was determined, were 49.1± 44.9 mg/L 1.1-396.2 mg/L , 5.8 ±3.1 1-19.4 mg/dL, 37±23.2 12-144 hours respectively.
