Does motherlessness cause mediastinitis? A Case Report Highlighting Adolescence Care Responsibilities and Biopsychosocial Considerations

Indroduction: In adolescence, personal care routines pass from parental control to the children. During this period, children develop independence in personal hygiene, clothing preferences, nutritional habits, and adhering to medical treatments. In case of lack of parental support and monitoring, it takes time for personal care habits to settle. Case: 16-year-old female patient was diagnosed with deep neck infection and descendant mediastinitis with massive pleural effusion. The patient discontinued her prior dental abscess treatment due to progressive dysphagia. The patient underwent surgery for neck exploration, insertion of a cervical drain, tracheostomy, and chest tube. Although tests for immunodeficiency were screened, no significant result was found. The patient was discharged on the 33rd day. Conclusion: In adolescence, challenges arise in maintaining effective clinical relationships, especially regarding risk-taking behaviors and adherence to medical treatment in the face of illness. Parental support and healthcare providers play a vital role in facilitating this transition by educating adolescents on self-care practices and increasing parental monitoring during this developmental phase. Also developing algorithms that address biopsychosocial integrity through artificial intelligence will contribute to identifying patients who are likely to be non-compliant with treatment.

Annesizliğin mediastinite yol açması: Ergenlik Dönemi Bakım Sorumluluklarını ve Biyopsikososyal Hususları Vurgulayan Bir Vaka Raporu

Öz

Giriş: Ergenlikte kişisel bakım rutinleri ebeveyn kontrolünden çocuklara geçer. Bu dönemde çocuklar kişisel hijyen, giyim tercihleri, beslenme alışkanlıkları ve tıbbi tedavilere uyum konusunda bağımsızlık geliştirirler. Ebeveyn desteği ve takibinin eksikliği durumunda kişisel bakım alışkanlıklarının oturması zaman alır. Olgu: 16 yaşında kadın hastaya ilerleyen disfaji nedeniyle önceki diş apsesi tedavisini bırakması üzerine derin boyun enfeksiyon kaynaklı, masif plevral efüzyonun eşlik ettiği mediastinit teşhisi konuldu. Hastaya boyun eksplorasyonu, servikal dren takılması, trakeostomi ve göğüs tüpü uygulandı. İmmün yetmezlik testleri taranmasına rağmen anlamlı bir sonuç bulunamadı. Hasta 33. günde sekelsiz taburcu edildi. Sonuç: Ergenlikte, özellikle hastalık karşısında risk alma davranışları ve tıbbi tedaviye uyum konusunda etkili klinik ilişkileri sürdürmede zorluklar ortaya çıkar. Ebeveyn desteği ve sağlık hizmeti sağlayıcıları, ergenlere öz bakım uygulamaları konusunda eğitim vererek ve bu gelişimsel aşamada ebeveyn denetimini artırarak bu geçişi kolaylaştırmada hayati bir rol oynar. Ayrıca, makine öğrenmesi aracılığıyla biyopsikososyal bütünlüğü ele alan algoritmalar geliştirmek, tedaviye uyumsuz olma olasılığı yüksek olan hastaların belirlenmesine katkıda bulunacaktır.
