Double ileocolic obstruction: Emergency management lessons from a resource-limited region

Introduction :Ileosigmoid knotting (ISK) is a rare and potentially life-threatening form of intestinal obstruction. Case report : We describe a 47-year-old patient from Tunisia, the second documented case nationally, presenting with acute abdominal pain and vomiting initially attributed to adhesive small bowel obstruction. Limited imaging resources in local hospital necessitated transfer to another facility, delaying surgical intervention. Exploratory laparotomy revealed a non-viable ISK , which was resected. Conclusion : This case highlights two key lessons: On a systemic level, shortages in diagnostic tools and intensive care capacity within public hospitals can negatively impact prognosis in rare but life-threatening emergencies. On a clinical level, emergency physicians and surgeons must maintain vigilance, and act decisively even in constrained environments. Greater awareness of rare entities such as ISK can lead to timely surgical intervention and potentially improve outcomes. 1. Atamanalp SS. Çift İleokolik Obstrüksiyon: Kaynakları Sınırlı Bir Bölgeden Acil Yönetim Dersleri

Öz

Giriş :İleosigmoid düğümlenme (İSD), nadir görülen ve potansiyel olarak yaşamı tehdit eden bir intestinal obstrüksiyon türüdür. Olgu Sunumu : Bu yazıda, Tunus’tan 47 yaşındaki bir hastayı—ülke genelinde bildirilen ikinci vakayı—sunuyoruz. Hasta, başlangıçta adezif ince barsak tıkanıklığına bağlanan akut karın ağrısı ve kusma ile başvurmuştur. Yerel hastanedeki görüntüleme olanaklarının kısıtlı olması, hastanın başka bir merkeze sevk edilmesini gerektirmiş ve cerrahi müdahalede gecikmeye yol açmıştır. Eksploratör laparotomi sonucunda, rezeksiyon gerektiren non-viabl bir İSD saptanmıştır. Sonuç: Bu olgu iki temel ders ortaya koymaktadır: Sistem düzeyinde, kamu hastanelerindeki tanısal araç eksiklikleri ve yoğun bakım kapasitesinin yetersizliği, nadir ancak yaşamı tehdit eden acil durumlarda prognozu olumsuz etkileyebilir. Klinik düzeyde ise acil hekimleri ve cerrahlar, kısıtlı koşullarda dahi yüksek düzeyde klinik uyanıklığı sürdürmeli ve kararlı şekilde hareket etmelidir. İSD gibi nadir patolojilere ilişkin farkındalığın artması, cerrahi müdahaleye zamanında karar verilmesini sağlayarak hasta sonuçlarını potansiyel olarak iyileştirebilir. 1. Atamanalp SS.
