Cauda Equina Syndrome in a 37-Week Pregnant Patient: Case Report

Low back pain is a very common complaint. There is an increase in the prevalence of these complaints with the changing anatomical and physiological balance with pregnancy. Although there are many factors in the etiology of low back pain, lumbar disc herniation (LDH) can be dangerous during pregnancy. This disease, which can lead to the development of cauda equina syndrome (CES), should be recognised early and treated effectively. Our case involves a 36-year-old patient who developed CES at 37 weeks and 2 days of pregnancy. The patient, with no prior history of LDH, presented with two days of back and right leg pain. Neurological examination confirmed the development of CES, which is a medical emergency. Based on this case, the pathophysiological processes involved in the development of CES, the patient's clinical presentation, and the emergency surgical intervention are discussed.

Öz

Bel ağrısı çok sık görülen bir şikayettir. Hamilelikle birlikte anatomik ve fizyolojik dengenin değişmesi nedeniyle bu şikayetlerin görülme sıklığı artmaktadır. Bel ağrısının etiyolojisinde birçok faktör bulunmakla birlikte, lomber disk hernisi (LDH) hamilelik sırasında tehlikeli olabilir. Kauda ekuina sendromunun (CES) gelişmesine yol açabilen bu hastalık erken teşhis edilmeli ve etkili bir şekilde tedavi edilmelidir. Vakamız, hamileliğinin 37. hafta 2. gününde CES gelişen 36 yaşındaki bir hastayı içermektedir. Daha önce LDH öyküsü olmayan hasta, iki gündür bel ve sağ bacak ağrısı şikayeti ile başvurdu. Nörolojik muayenesinde, tıbbi bir acil durum olan CES geliştiği görüldü. Bu vaka temelinde, CES gelişiminde rol oynayan patofizyolojik süreçler, hastanın klinik tablosu ve acil cerrahi müdahale tartışılmıştır.
