Traumatic Posterior Tibial Tendon Rupture After Bimalleolar Fracture: Case Report and Review of the Literature

Introduction: Rupture of the posterior tibial tendon associated with closed ankle fractures is seen rarely been and such cases in the literature are usually reported as case reports. Case Report: Bimalleolar fracture was determined in a 49 years old man after a fall from stairs. Management was consisted of open reduction-internal fixation of the fractures and primer repair of the tibialis posterior tendon. After 12 months follow-up, the patient had adequate ankle range of motion and comfortable walking. Significant major complication was not noted. Conclusion: However it is a rare condition, rupture of tibialis posterior tendon should not be overlooked after ankle fractures. These ruptures can be diagnosed only at surgery since pain inhibits clinical examination. Misdiagnosed and untreated cases can cause major complications such as ankle deformity, ankle joint arthrosis and chronic pain. In this case report, we aimed to share our unique experience of a posterior tibial tendon rupture associated with ankle fracture and to present a comprehensive review of the literature

Bimalleoler Kırık Sonrası Travmatik Posterior Tendon Rüptürü: Olgu sunumu ve literatür derlemesi

Öz

Giriş: Kapalı ayak bileği kırığıyla ilişkili ve birlikte posterior tibial tendon rüptürü nadir görülür ve bu durum literatürde genellikle olgu sunumu şeklinde rapor edilmiştir. Olgu Sunumu: 49 yaşındaki erkekte, merdivenden düşme kazasıyla ilgili ayak bileğinde bimalleoler kırık tespit edildi. Tedavisinde; kırığın açık redüksiyon-internal fiksasyonu ve posterir tibial tendonun primer tamiri yapıldı. Bu olguda yaralanmadan 12 ay sonra hastanın; ayak bileği eklem hareketleri yeterliydi ve rahat yürüyebiliyordu. Major komplikasyon gözlenmedi. Sonuç: Seyrek görülmesine rağmen, ayak bileği kırıkları sonrası posterior tibial tendon rüptürü gözden kaçırılmamalıdır. Bu yaralanmalara, ağrının klinik muayeneyi engellemesi nedeniyle cerrahi esnasında tanı konabilir. Tanısı atlanmış ve tedavi edilmemiş bu olgular, ayak bileği deformitesi, ayak bileği eklemi artrozu ve kronik ağrı gibi major komplikasyonlarla sonuçlanabilir. Bu olgu sunumunda; ayak bileği travmasıyla ilişkili posterior tibial tendon rüptürü olgusundaki deneyimlerimizi paylaşmayı ve kapsamlı bir literatür gözden geçirmesi sunmayı amaçladık.
