Introduction: As a result of hyperextension of the axis bone (C2), fracture occurs in posterior elements of the same bone. It causes a forward shift of spine C3 on spine C2 (traumatic spondylolisthesis). Because this mechanism is the cause of death during hanging, this name was given. In sudden braking and collisions at traffic accidents, it occurs as a result of the head hitting the windshield when the neck is in hyperextension   Case Report: A 68-year-old male patient presented to our emergency unit because of neck pain after an intravehicular traffic accident. Neurologic evaluation of the patient revealed no deficit. X-ray showed a dislocation of 2 mm due to fracture in the posterior elements at the C2 level. There was no angulation accompanying the dislocation. Based on the radiologic evaluation, the case was diagnosed with type 1 hangman’s fracture. The patient was prescribed 6-month collar use. Eventually, the fracture exhibited fusion, and the dislocation resolved.   Conclusion: In the literature, there is not much visual evidence involving such a fusion of type 1 hangman’s fracture achieved only by collar use. We presented this case because of its rarity and achievement of a positive outcome with only conservative treatment.
