Diurnal Ritmin Term ve Postterm Gebeliklerde Fetal Biyofizik Profil Üzerine Etkisi

Öz

Amaç: Biyofizik profil (BFP), fetal iyilik hâlinin değerlendirilmesinde kullanılan, non-invaziv ve güvenilir bir testtir. Bu çalışmada, term ve postterm gebeliklerde BFP parametrelerinin gün içi (diurnal) değişimlerini değerlendirmek amaçlanmıştır. Gereç ve Yöntem: Bu prospektif gözlemsel çalışmaya, 75 term ve 75 postterm gebe dahil edilmiştir. Sistemik hastalığı, fetomaternal risk faktörleri bulunanlar, sigara kullananlar ve diurnal ritmi etkileyebilecek sosyal ya da mesleki alışkanlıkları olanlar dışlanmıştır. Katılımcıların BFP ve non-stres test (NST) değerlendirmeleri sabah (08.00–10.00) ve akşam (20.00–22.00) saatlerinde gerçekleştirilmiştir. Elde edilen veriler hem grup içi hem gruplar arası olarak analiz edilmiştir. Bulgular: Fetal hareket, solunum, NST bulguları ve ortalama BFP skorları açısından, akşam saatlerinde yapılan değerlendirmelerde sabah saatlerine kıyasla anlamlı düzeyde daha yüksek skorlar elde edilmiştir. Bu durum hem genel hasta grubunda hem de term ve postterm gruplarda ayrı ayrı gözlenmiştir. Sabah saatlerinde postterm gebelerde ortalama BFP skorları, term gebelere göre anlamlı düzeyde daha düşük bulunmuştur. Fetal solunum sayısı postterm grupta daha az olmakla birlikte istatistiksel olarak anlamlı değildir. Akşam ölçümlerinde de postterm grubun ortalama BFP skorları daha düşük bulunmuştur. Sonuç: Diurnal ritmin fetal biyofizik parametreler üzerinde anlamlı etkileri bulunmaktadır. Hormon düzeylerinin kantitatif olarak değerlendirildiği daha geniş örneklemli çalışmalarla bu etkinin biyolojik mekanizmaları daha net ortaya konabilir.

The Effect of Diurnal Rhythm on Fetal Biophysical Profile in Term and Postterm Pregnancies

Öz

Aim: The biophysical profile (BPP) is a non-invasive and reliable test used to assess fetal well-being. This study aimed to evaluate the diurnal variations in BPP parameters in term and postterm pregnancies. Materials and Methods: This prospective observational study included 75 term and 75 postterm pregnant women. Participants with systemic diseases, fetomaternal risk factors, smoking habits, or social/occupational conditions that could affect the diurnal rhythm were excluded. BPP and non-stress test (NST) evaluations were performed in the morning (08:00–10:00) and evening (20:00–22:00). The collected data were analyzed both within groups and between groups. Results: Evening assessments revealed significantly higher scores for fetal movements, breathing, NST findings, and mean BPP scores compared to morning evaluations. This pattern was observed both in the overall study population and within term and postterm groups separately. In the morning measurements, mean BPP scores were significantly lower in postterm pregnancies than in term pregnancies. Although the number of fetal breathing was lower in the postterm group, the difference was not statistically significant. In the evening measurements, the postterm group also had lower mean BPP scores. Conclusion: Diurnal rhythm has a significant impact on fetal biophysical parameters. Further large-scale studies with quantitative assessment of hormonal levels are needed to clarify the underlying biological mechanisms.
