Comparison of Pregnancy Outcomes Among Pregnant Women with Epilepsy Diagnosis and Perinatal Outcomes with Non-Epileptic Pregnant Women Without Epilepsy Diagnosis: A Retrospective Study

Öz

Introduction: Research; This study was planned retrospectively in order to compare the problems experienced by women diagnosed with epilepsy during pregnancy, seizure status, drug use, and maternal and fetal outcomes with non-epileptic pregnant women. Materials and Methods: The study was carried out retrospectively on the records of pregnant women with and without epilepsy. The total sample group in the study consisted of 445 pregnant women (49 in the study group, 396 in the control group). The data of the study were collected by examining all birth records in the Obstetrics and Gynecology Clinic of a Medical Faculty Hospital in Konya between March 2015 and February 2020. The data of the study were recorded using the data collection form. Results: No statistically significant difference was found between the epileptic and non-epileptic pregnant women included in the study in terms of age, gravida and parity (p>0.05). It was determined that epileptic pregnants made their first doctor visits later in pregnancy, while the total number of antenatal visits was lower (p<0.05). It has been determined that the antenatal and postnatal complications of epileptic pregnant women are at low levels, the drug is used at a high rate, and although there is a small increase in seizures with pregnancy, this is not statistically significant. Conclusion: Epileptic pregnant women do not have worse antenatal, natal and postnatal outcomes compared to non-epileptic pregnant women. The importance of antenatal follow-up should be explained to epileptic pregnant women starting from the preconceptional period, and their awareness should be increased about the importance of using drugs that can prevent epileptic seizures and closely following dose settings.

Epilepsi Tanılı Gebelerin Gebelik Sonuçları ve Epilepsi Tanısı Olmayan Gebeler ile Perinatal Sonuçların Karşılaştırılması: Retrospektif Araştırma

Öz

Amaç: Araştırma; epilepsi tanısı almış kadınların gebelikte yaşadığı sorunlar, nöbet durumları, ilaç kullanım durumları ve epileptik olmayan gebeler ile maternal ve fetal sonuçlarının karşılaştırılması amacıyla retrospektif olarak planlanmıştır. Gereç ve Yöntem: Araştırma, epilepsi tanısı olan ve epilepsi tanısı olmayan gebelerin kayıtları üzerinde retrospektif olarak gerçekleştirilmiştir. Araştırmada toplam örneklem grubunu 445 gebe (çalışma grubu 49, kontrol grubu 396) oluşturmuştur. Araştırmanın verileri Mart 2015-Şubat 2020 tarihleri arasında Konya ilinde yer alan bir Tıp Fakültesi Hastanesinin Kadın Doğum Kliniğindeki tüm doğum kayıtları incelenerek toplanmıştır. Araştırmanın verileri veri toplama formu kullanılarak kayıt altına alınmıştır. Bulgular: Çalışmaya dahil edilen epileptik olan ve olmayan gebeler arasında yaş, gravida ve parite açısından istatistiksel olarak anlamlı bir farklılık bulunmamıştır (p>0.05). Epileptik gebelerin gebelikteki ilk doktor ziyaretlerini daha geç yaparken, toplam antenatal ziyaret sayılarını daha az olduğu saptanmıştır (p<0.05). Epileptik gebelerin antenatal ve postnatal komplikasyonlarının düşük düzeylerde olduğu, yüksek oranda ilaç kullanıldığı ve gebelikle birlikte nöbetlerde çok az miktarda artış yaşansa da bunun istatistiksel açıdan anlamlı olmadığı tespit edilmiştir. Sonuç: Epileptik gebeler epileptik olmayan gebeler ile karşılaştırıldığında daha kötü antenatal, natal ve postnatal sonuçları bulunmamaktadır. Epileptik gebelere prekonsepsiyonel dönemden başlayarak antenatal izlemin önemi anlatılmalı, epileptik nöbetleri engelleyebilecek ilaç kullanımının, doz ayarlarının yakından takip edilmesinin önemi konusunda farkındalıkları artırılmalıdır.
