Professional Fears and Coping Responses Among Midwives During Childbirth Care in Türkiye: A Cross-Sectional Study

Öz

Aim: This study aimed to examine the professional fears experienced by midwives during childbirth care and the coping responses used to manage these fears. Method: This descriptive cross-sectional study was conducted with 394 midwives working in Türkiye. Data were collected using a structured online questionnaire developed by the researchers, consisting of a sociodemographic and professional characteristics form, questions assessing professionally challenging situations and fears experienced during childbirth care, and an open-ended section evaluating coping responses. Descriptive statistics, Chi-square, One-Way ANOVA, Welch, and Kruskal-Wallis tests were used for data analysis. Results: The highest fear score reported by midwives was fear of performing a harmful intervention (9.55±1.19), followed by fear of maternal death (9.30±1.53), causing a negative birth experience(8.47±2.02), and neonatal death (8.44±2.29). Significant associations were found between several fear domains and age, years of professional experience, workplace, and birth experience (p<0.05). Most midwives (67.8%) reported that they did not use any coping strategy to manage their fears. Among those who used coping strategies, improving professional knowledge (7.9%), practicing meditation (7.9%), and engaging in social activities (5.1%) were the most commonly reported approaches. Conclusions: Midwives experience considerable professional fears during childbirth care, particularly regarding harmful interventions, maternal death, and neonatal death. Fear levels are associated with both professional and individual characteristics, while the majority of midwives reported not using any specific coping strategies. Supportive institutional policies and professional support mechanisms may help improve midwives’ psychological well-being and professional functioning.
