Romatoid Artritli Hastalarda Kuadriseps Kasının Mekanik Özellikleri ve Psikolojik Durumun Değerlendirilmesi

Öz

Amaç: Bu çalışmanın amacı, romatoid artritli (RA) hastalarda bilateral kuadriseps femoris kasının viskoelastik özelliklerini ve psikolojik durumlarını değerlendirmektir. Yöntem: Bu tanımlayıcı kesitsel çalışmaya romatoid artrit (RA) tanısı almış 26 hasta dahil edildi. Katılımcıların klinik özellikleri ve temel demografik verileri standartlaştırılmış değerlendirme formları aracılığıyla kaydedildi. Kuadriseps kasının istirahat osilasyon frekansı, dinamik sertlik, elastikiyet, gevşeme süresi ve creep gibi mekanik ve viskoelastik özellikleri, her iki alt ekstremitede miyotonometri (MyotonPRO) kullanılarak değerlendirildi. Anksiyete ve depresyonu içeren psikolojik durum ise Hastane Anksiyete ve Depresyon Ölçeği (HADS) ile değerlendirildi. Bulgular: Katılımcıların yaş ortalaması 54,27 ± 11,13 yıl olup, hastaların %80,8’i kadındı. Ortalama hastalık süresi 154,62 ± 122,43 ay olarak bulundu. HADS Anksiyete ve HADS-Depresyon puan ortalamaları sırasıyla 8,42 ± 4,53 ve 7,04 ± 3,88 idi. Kuadriseps femoris kasının mekanik özellikleri dominant ve non-dominant ekstremiteler arasında benzer bulundu. Osilasyon frekansı dominant tarafta 11,24 ± 1,94 Hz, non-dominant tarafta ise 11,53 ± 2,12 Hz olarak ölçüldü. Dinamik sertlik değerleri dominant ve non-dominant taraflarda sırasıyla 192,28 ± 54,76 N/m ve 203,36 ± 57,47 N/m olarak bulundu. Frekans, sertlik, decrement, gevşeme süresi ve creep parametreleri açısından ekstremiteler arasında anlamlı fark saptanmadı (p > 0,05). Sonuçlar: Romatoid artritli hastalarda kuadriseps femoris kasının mekanik özelliklerinde değişiklikler ve klinik açıdan anlamlı eşik değerlere yaklaşan anksiyete ve depresyon düzeyleri gözlenmiştir. Bulgular, RA’nın yalnızca eklemleri değil, aynı zamanda iskelet kasının intrinsik mekanik özelliklerini de etkilediğini göstermektedir. RA yönetiminde kas-iskelet sistemi ve psikolojik faktörleri birlikte ele alan bütüncül rehabilitasyon yaklaşımlarının yararlı olabileceği düşünülmektedir.

Evaluation of Quadriceps Muscle Mechanical Characteristics and Psychological Status in Patients With Rheumatoid Arthritis

Öz

Aim: This study aimed to investigate the bilateral viscoelastic properties of the quadriceps femoris muscle and the psychological status of patients with rheumatoid arthritis (RA). Method: Twenty-six patients diagnosed with RA participated in this descriptive cross-sectional trial. Clinical and baseline demographic matrices were gathered via standardized forms. Bilateral assessments of the quadriceps muscle—focusing on resting oscillation frequency, dynamic stiffness, elasticity, relaxation phase, and creep—were dynamically captured via myotonometry (MyotonPRO). Psychological distress parameters encompassing anxiety and depression traits were quantified utilizing the Hospital Anxiety and Depression Scale (HADS). Results: The mean age of the participants was 54.27 ± 11.13 years, and 80.8% were female. The mean disease duration was 154.62 ± 122.43 months. Mean HADS-Anxiety and HADS-Depression scores were 8.42 ± 4.53 and 7.04 ± 3.88, respectively. The quadriceps femoris muscle demonstrated comparable mechanical properties between dominant and non-dominant limbs. Oscillation frequency was 11.24 ± 1.94 Hz on the dominant side and 11.53 ± 2.12 Hz on the non-dominant side. Dynamic stiffness values were 192.28 ± 54.76 N/m and 203.36 ± 57.47 N/m for the dominant and non-dominant sides, respectively. No significant differences were observed between limbs regarding frequency, stiffness, decrement, relaxation time, or creep (p > 0.05). Conclusions: Patients with long-standing RA exhibited alterations in quadriceps femoris muscle viscoelastic properties together with anxiety and depression scores approaching clinically relevant thresholds. The findings suggest that RA affects not only joint structures but also the intrinsic mechanical properties of skeletal muscle. Comprehensive rehabilitation approaches addressing both musculoskeletal and psychological factors may be beneficial in the management of RA
