Causal Mapping and Priority Setting for Healthcare Blockchain Adoption

Blockchain adoption in healthcare depends on how technical, organizational, and environmental conditions interact in practice. To produce a decision-oriented view rather than a descriptive list, the Technology Organization Environment framework is combined with fuzzy DEMATEL and DANP. Fourteen healthcare managers provided pairwise influence ratings across ten factors that were encoded as triangular fuzzy numbers, defuzzified, and normalized before computing the total relation matrix. The analysis separates drivers from receivers and then translates the causal map into steady state priorities. Compliance and Security emerge as upstream drivers that influence the direction toward Interoperability and Economic Incentives. The DANP weights concentrate on these two outcomes, indicating where benefits accumulate when upstream levers are activated. The main implication is operational: start with compliance by design and security controls, backed by explicit leadership sponsorship and organizational readiness. Then, target interoperability and measurable economic value while reducing complexity and improving scalability. The workflow is reproducible in typical healthcare settings and can be replicated with the materials described in the paper.
